Case Manager - HEAP & Emergency Services

Are you organized, compassionate, and driven to make a real impact in your community? We are seeking a highly motivated Case Manager to serve individuals and families facing crisis through our HEAP and Emergency Services programs in Ashland County.

This is an opportunity to do meaningful work every day -  helping neighbors maintain utilities, prevent homelessness, and move toward long-term stability.

What You'll Do:

•          Conduct thorough client intake and needs assessments

•          Guide applicants through HEAP and Emergency Services processes

•          Coordinate community resources and supportive services

•          Maintain accurate case documentation and required reports

•          Approve applications within funding guidelines

•          Manage multiple cases with professionalism and attention to detail
