We have an excellent Full-Time opportunity for a dynamic *Physical Therapist (PT)* to care for our residents in the comfort of their own homes. Must have a physical therapy license in the State of Missouri, excellent communication and computer skills as well as reliable transportation. The qualified candidate may also possess a Master of Physical Therapy or Doctorate. We ask our dear staff to accomplish the following in the home:
* Evaluate and assess patients to develop individualized treatment plans
* Provide therapeutic exercises, manual therapy, and functional training
* Monitor patient progress and adjust care plans as needed
* Educate patients and families on home exercise programs and recovery strategies
* Document patient care accurately and timely in EMR system
* Collaborate with physicians, nurses, and interdisciplinary team members
* Other duties as assigned
*Qualifications:*
* Master’s degree (minimum) in Physical Therapy from an accredited program
* Licensed Physical Therapist in the state of Missouri
* National Board for Certification in Physical Therapy
