This
full-time
role involves providing exceptional care in an outpatient setting using the latest evidence-based techniques. We believe in collaborative work with other healthcare professionals and colleagues to ensure dynamic treatment for our clientele. Assess patient goals and limitations to develop a therapeutic plan of care and therapy timeline Carry out therapeutic interventions using the latest evidence-based techniques that appropriately address a patient’s specific needs and concerns Document initial evaluations, daily notes, progress notes, and discharge summaries using Raintree electronic medical records Maintain patient records in accordance with federal and state regulations and company policy to ensure excellent standard of care Provide regular updates to patients, physician’s and other therapists involved in the patient’s care Communicate effectively with team of healthcare providers and colleagues to ensure an exceptional continuum of care Educate patients, families and caregivers on a home exercise program that complements therapy sessions Participate in continuing education and other professional development opportunities to remain knowledgeable about new and emerging therapies.
