We are seeking a dedicated and skilled Physical Therapist to join our team. The ideal candidate will have a strong background in physical therapy with a focus on geriatrics, acute care, and orthopedics. This role involves assessing patient needs, developing individualized treatment plans, and providing high-quality care to enhance patient mobility and overall health. The Physical Therapist will work collaboratively with other healthcare professionals to ensure comprehensive discharge planning and effective home community care.
*Duties*
* Conduct thorough evaluations of patients' physical abilities and limitations.
* Develop and implement personalized treatment plans based on assessment findings.
* Provide hands-on therapy to improve patients' strength, flexibility, and mobility.
* Educate patients and their families on exercises, techniques, and home care strategies.
* Collaborate with interdisciplinary teams to coordinate discharge planning and follow-up care.
* Maintain accurate documentation of patient progress and treatment outcomes in compliance with healthcare regulations.
* Stay updated on advancements in physical therapy practices, particularly in geriatrics, acute care, and orthopedics.
*Requirements*
* Valid state license to practice as a Physical Therapist.
* Strong knowledge of human physiology and anatomy.
* Experience in physical therapy settings such as clinics or hospitals is preferred.
* Familiarity with systems related to discharge planning and home community care.
* Excellent communication skills for effective interaction with patients, families, and healthcare teams.
* Ability to work independently as well as part of a collaborative team environment.
* A passion for improving the quality of life for patients through rehabilitative care.

Job Type: Full-time
Pay: $48.13 - $52.38 per hour
