#1 In Home Care is seeking an experienced, dynamic, and committed Direct Service Worker (DSW) Case Manager to join its leadership team. We’re looking for a proactive professional who understands home care regulations, promotes compliance and quality, and is passionate about mentoring and developing others.
As a DSW Supervisor, you will shape a culture of teamwork, respect, and accountability — ensuring every DSW feels supported and every client receives the highest standard of care.
Key Responsibilities
- Supervise and mentor DSWs to ensure safe, effective, and compliant care.
- Conduct regular field visits and unannounced supervisory checks to evaluate quality and performance.
- Ensure accurate documentation of service notes, client records per state and federal regulations, and timesheet approvals.
- Provide coaching, performance feedback, and ongoing development support to DSWs.
- Maintain adequate staffing and provide on-call coverage when needed.
- Collaborate with clients, families, support coordinators, and other providers to ensure continuity and satisfaction of care.
- Uphold agency policies and contribute to organizational growth through innovation, accountability, and integrity.
Minimum Qualifications
- High school diploma or equivalent (associate or bachelor’s degree preferred).
- Minimum 5+ years' experience in home care, caregiving, case management, or a related people-focused role.
- Previous experience supervising, leading, or mentoring others in a home care agency, nursing home, or similar setting is strongly preferred.
- Knowledge of Louisiana Department of Health (LDH) regulations and DSW standards highly preferred.
- Excellent leadership, communication, and organizational skills.
- Proficiency with Microsoft and Google business software; LASRS experience preferred.
- Current CPR/First Aid certification.
- Valid driver’s license, reliable transportation, and proof of insurance.
- Ability to travel for site visits and work flexible hours when needed.

- Requires travel and occasional work outside standard office hours.
