*Job Specific Requirements:*
* To possess the knowledge, skills, and current competence and proficiency in the appropriate pharmaceutical care and Medication Therapy Management services assigned based on the scope of service.
* To participate in decisions regarding pharmaceutical care services within their assigned scope to include:
a. assessing the patient's general pharmaceutical needs and their ability to follow the prescribed therapy.
b. assess the patient’s therapy from the standpoint of the patient’s diagnosis, dosing plan, administration rates, routes of administration, drug-drug and drug food interactions, and possible adverse effects.
c. create and maintain each patient’s pharmacy Plan of Care and assure that the patient/caregiver/family has participated in the development of the Plan of Care and is fully aware of the Plan of Care.
d. planning for the provision of pharmaceutical services.
e. participating in the overall Plan of Care for each patient as well as working collaboratively with the patient’s nursing services and referring physician.
* Receive physician/prescriber prescription orders and evaluate the appropriateness of these orders for dosage, drug choice in relationship to the patient’s diagnosis, potential drug interactions, route of administration and stability.
* Ensure that the patient database is complete for current height, weight, current and past medical history, clinical laboratory results, and concurrent medications (including over-the-counter, herbal, or home remedy medications, as well as all nutritionals).
* Assist in the maintenance and upkeep of all necessary paperwork and documentation required by state and federal agencies as well as accrediting bodies (i.e. JCAHO, ACHC).
* Ensure that when necessary, clinical laboratory tests have been ordered and the results are obtained and discussed with the nurse and the patient’s physician(s).
* Assess the clinical laboratory test results in relationship to the safety and effectiveness of the patient’s medication and nutritional therapies.
* Assist the physician, nurse, and patient with clinical and technical support.
* Assist the pharmacist-in-charge in implementing new policies and procedures.
* Assist in the training and supervision of pharmacy technicians, pharmacy interns, laboratory technicians and other pharmacy personnel.
* Directly supervise all sterile compounding and the quality assurance programs (i.e. validation testing, environmental testing, end product testing).
* Document all communications with physicians, nurses, other healthcare professionals, and patients/caregivers in the patient’s medical record.
* Ensure safety to patients and personnel by exercising good pharmacy practice.
* Participate in the process of providing drug therapy instructions and education to patients/caregivers.
* Participate in inter-disciplinary patient care conferences.
* Assist in providing in-service programs to nursing personnel on all aspect of medication therapy.
* Assist in training pharmacy technicians, nurses, and when appropriate patients/caregivers in the proper storage, administration, and adverse effect monitoring, including monitoring for infections. Assure that nurses and patients/caregivers are trained properly in appropriate sterile compounding methods.
* Assure that any collaborative practice agreements are closely followed and that all pharmacy personnel and nurses understand the therapy management protocols.
* Is expected to perform other tasks as determined by the pharmacist-in-charge/pharmacy manager.
* Exposure to hazardous chemicals/waste may occur, therefore since these agents may cause harmful effects, all individuals will have medical examination annually.
* If an individual is actively trying to conceive or they are pregnant, they will avoid handling any hazardous chemicals/waste (i.e. cytotoxic agents).
* Pharmacist will meet the State Pharmacy Board Continuing Education requirements and a copy of the documentation for each completed ACPE approved C.E. program will be kept in the individual’s personnel file.
* Responsible for ensuring a high level of patient satisfaction and quality including trouble shooting patient issues and proactively participating in the corporate Quality Improvement Program
* Must maintain an active and unrestricted pharmacy license(s) in current state(s) of practice.
*Job Requirements:*
* Maintain compliance with all company policies and procedures and ensures services are provided in a manner consistent with established standards of practice, federal and state regulations, and accreditation standards.
* Excellent verbal and written communication skills, including ability to effectively communicate with internal and external customers

* Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.

* Maintains confidentiality of patient and competitive information.
* Participates in the AvevoRx Quality Assurance Program.
* Completes work on schedule.
* All other duties as assigned.
*Physical Requirements: *
* Must be able to lift and carry up to 75 lbs.
* Standing, walking, and bending for long period of time.
* Must be able to talk, listen and speak clearly on telephone.
* Immunizations as required and TB test/screen annually.
*Required Qualifications: *
* Graduation from an accredited College of Pharmacy with current licensure in state(s) of practice.
* Willing to obtain license in all states, which are serviced by the facility or organization.
* Minimum 1 to 2 years’ experience in hospital or other infusion care provider pharmacy.
* Completed an aseptic technique and sterile compounding program/course (e.g., ASHP program) and/or completed employer training program.
* All candidates must pass a federal background check and are subject to drug testing.
*Preferred Qualifications: *
* Experience with CPR+.
* Experience with maintaining and managing a clean room under USP 797 standards.
* Bi-lingual (fluent English and Spanish) candidates.
* Hold multiple state pharmacy licenses, and/or willingness to obtain additional state RPH licenses.
Job Type: Full-time
Pay: $60.00 - $65.00 per hour
