Hospice Aide
provides compassionate personal care and supportive services to patients in their place of residence. This role assists individuals who are nearing end of life, helping maintain dignity, comfort, and quality of life. The Hospice Aide is an essential member of the care team and delivers care in alignment with the patient's individualized Plan of Care. Provide assistance with at least 2 of the following ADL's including personal hygiene • Incidental household services, which are essential to patient health care • Will follow the Plan of Care (Instruction Sheet) as instructed and make appropriate updates during scheduled visits • Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care Must be a certified nurse's aide (CNA). State Tested Nursing Assistant (STNA) for Ohio High school graduates or GED preferred Demonstrate the ability to work in field situations Preferred candidates will have in-home and/or hospice experience
