*Responsibilities*
* Rates and hours may vary by visit type.
* Conduct comprehensive patient assessments to evaluate physical function, mobility, and pain levels using physiology knowledge and medical terminology.
* Develop and implement individualized care plans that incorporate therapeutic exercises, trigger point therapy, and other physiotherapy modalities tailored to each patient’s condition.
* Provide hands-on treatment for various conditions including orthopedics, geriatrics, pediatrics, and post-surgical rehabilitation.
* Manage discharge planning by coordinating with healthcare teams to ensure seamless transitions from hospital to home or community care settings.
* Document patient progress accurately within electronic medical records (EMR systems), maintaining detailed records of treatments administered and outcomes achieved.
* Educate patients and their families on injury prevention strategies, home exercises, and ongoing care requirements to support long-term health goals.
* Collaborate with multidisciplinary teams to optimize patient care plans and ensure comprehensive treatment delivery across inpatient, outpatient, hospital experience, or home & community care environments.
*Skills*
* Strong knowledge of anatomy and physiology to accurately diagnose and treat musculoskeletal and neurological conditions.
* Proven experience in discharge planning, acute care, inpatient hospital settings, outpatient clinics, or home health environments.
* Expertise in physiotherapy techniques such as trigger point therapy and exercise prescription for orthopedics and geriatrics.
* Excellent understanding of medical terminology relevant to physical therapy practice.
* Ability to utilize EMR systems efficiently for documentation and communication purposes.
* Familiarity with care plans development for diverse populations including pediatrics and older adults.

Pay: $80.00 - $120.00 per hour
Expected hours: 10 – 20 per week
