South Mountain Post Acute is a *5-Star Skilled Nursing Facility* in South Phoenix seeking a *Licensed Physical Therapist* to join our in-house therapy team.
This is an opportunity to practice *true patient-centered therapy* in a stable, supportive environment where your clinical judgment is valued and your growth is encouraged.

*What you’ll do:*
* Evaluate patients within 48 hours of referral
* Develop and implement individualized treatment plans
* Supervise PTAs and rehab aides
* Collaborate with interdisciplinary care teams
* Participate in care conferences and discharge planning
* Document treatments and progress in EMR
* Coordinate adaptive and durable medical equipment
*Qualifications:*
* Active Physical Therapist license
* All experience levels welcome , new grads encouraged
