*Overview*
We are seeking a dedicated and skilled Physical Therapist to join our home health team. The ideal candidate will possess a strong background in patient care and a comprehensive understanding of physiotherapy practices. This role involves working with patients in there home home. The Physical Therapist will be responsible for developing and implementing effective treatment plans to enhance patient recovery and improve quality of life.
*Responsibilities*
* Conduct thorough assessments of patients to determine their physical therapy needs.
* Develop individualized care plans based on patient evaluations, goals, and progress.
* Implement treatment strategies that may include manual therapy, exercise programs, muscle strength, mobility, gait , ROM
* Collaborate with interdisciplinary teams to ensure comprehensive patient care
* Educate patients and their families on treatment plans, exercises, and self-care strategies to promote recovery.
* Document patient progress accurately using EMR systems and maintain compliance with medical terminology standards.
* Participate in discharge planning for patient

*Qualifications*
* Valid state licensure as a Physical Therapist is required.
* Strong knowledge of anatomy and physiology relevant to physical therapy practices.
* Experience in various settings including inpatient, outpatient clinics, home health care, and acute care environments is preferred.
* Familiarity with discharge planning processes and the development of effective care plans.
* Proficiency in EMR systems for documentation and patient management.
* Excellent communication skills for interacting with patients, families, and healthcare professionals.

Pay: $135.00 - $145.00 per hour
