We are seeking a dedicated and energetic Licensed Physical Therapist to join our dynamic home healthcare team. In this vital role, you will deliver comprehensive physiotherapy services to diverse patient populations in the comfort of their homes. Serving area include Wasilla, Palmer, Houston, Big Lake, Willow, Sutton, Talkeetna in the state of Alaska. Your expertise will help people to improve mobility, reduce pain, and enhance overall quality of life for patients of all ages. You will develop personalized care plans, perform detailed assessments, and collaborate with multidisciplinary teams to ensure optimal patient outcomes. This position offers a rewarding opportunity to make a meaningful difference through expert patient care and innovative therapy techniques.
*Duties*
* Conduct thorough evaluations of patients’ physical conditions using physiology and anatomy knowledge to inform treatment strategies
* Develop and implement individualized care plans tailored to each patient’s needs, goals, and medical history
* Administer physiotherapy treatments including trigger point therapy, orthopedics, geriatrics, pediatrics, and home & community care interventions
* Monitor patient progress regularly and adjust care plans accordingly to maximize recovery and functional improvement
* Assist with discharge planning by coordinating follow-up care, home modifications, or community resources as needed
* Document all patient interactions accurately within electronic medical record (EMR) systems in compliance with healthcare regulations
* Collaborate with physicians, nurses, and other healthcare professionals to ensure seamless patient care across inpatient and outpatient settings
*Qualifications*
* Valid licensure as a Physical Therapist in the the state of Alaska in good standing.
* Previous experience in home health settings is preferred.
* In-depth knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques such as trigger point therapy.
* Familiarity with EMR systems for efficient documentation and treatment tracking.
* Previous experience working with diverse populations including pediatrics and geriatrics in various care environments is preferred.
* Strong understanding of discharge planning processes and community-based care coordination.
* Good communication skills to effectively educate patients on treatment plans and self-care strategies.

Pay: $42.00 - $65.00 per hour
Expected hours: 1 – 10 per week
