*What You’ll Do as an LPN Team Lead*
* Provide hands-on nursing care while supporting a high standard of resident safety and comfort
* Lead, mentor, and supervise Certified Nursing Assistants (CNAs)
* Administer medications and treatments accurately and efficiently
* Monitor resident conditions and promptly report changes
* Complete documentation for admissions, transfers, and discharges
* Communicate clearly with residents, families, and interdisciplinary teams
* Maintain compliance with infection control, safety protocols, and state regulations
* Provide compassionate end‑of‑life care when needed
*Qualifications*
* Graduate of an accredited Practical Nursing program
* Active *New York State LPN license*
* Current CPR certification
* Strong communication, leadership, and teamwork abilities
* Ability to thrive in a fast-paced skilled nursing environment
