Full-Time | Skilled Nursing Facility
*Crystal Cove Post Acute* is seeking a compassionate, motivated, and clinically strong *Physical Therapist (PT)* to join our interdisciplinary team. We are a fast-growing skilled nursing facility focused on quality outcomes, collaborative care, and restoring residents to their highest level of independence.
This is an excellent opportunity for a therapist who enjoys working in a dynamic post-acute setting and values teamwork, clinical excellence, and resident-centered care.
*Position Summary*
The Physical Therapist evaluates residents, develops individualized treatment plans, and delivers evidence-based therapy services to improve mobility, strength, safety, and functional independence. The PT collaborates closely with nursing, rehab staff, and the IDT to support safe discharges and optimal outcomes.
*Key Responsibilities*
* Perform PT evaluations, assessments, and re-assessments
* Develop and implement individualized treatment plans
* Provide skilled therapeutic interventions and functional mobility training
* Collaborate with physicians, nursing, case management, and rehab team
* Document services accurately in EMR in compliance with CMS and state regulations
* Participate in care planning meetings and discharge planning
* Educate residents, families, and staff on mobility and safety strategies
*Qualifications*
* Licensed Physical Therapist in the State of Washington (required)
* Graduate of an accredited Physical Therapy program (DPT or equivalent)
* Skilled Nursing Facility or post-acute experience preferred
* Strong clinical reasoning and time-management skills
* Knowledge of Medicare Part A and rehabilitation best practices
* Excellent communication and team collaboration abilities
