We are seeking a licensed Physical Therapist (PT) for a position PRN, for in one of our long- term health facilities throughout Indiana. Our partnership with Magnolia Health Systems means we can offer you job security, professional advancements and the best working conditions around.

Indianapolis and Hendricks County Area
*Requirements for a Physical Therapist (PT):*
* Graduate of an accredited Physical Therapy Program. The program must be accredited by the American Physical Therapy Association (APTA)
* Must be currently licensed to practice in the state of Indiana
* Previous experience in home health or in a health care setting is recommended
*Responsibilities for a Physical Therapist (PT):*
* Follow receipt of a doctor's referral, the therapist is responsible for evaluating the patient within 24 hours of the written referral
* Therapist is responsible for planning, administering and supervising the PTA in an appropriate treatment plan
* Will maintain an accurate daily record of treatments given to patients
* Will provide in-service education to facilities, supportive staff, students and community
* Documentation in NetHealth; will train

Job Type: Part-time
Pay: From $50.00 per hour
