Position Summary
The Clinical Coordinator plays a key role in supporting families receiving FSCD Specialized Services by helping them navigate supports, maintain required documentation, and stay connected with relevant community systems. They do not provide clinical services or intervention strategies but serves as a liaison between the family, the agency, FSCD, and other external supports such as schools, PDD, and healthcare providers. Theey ensure that the administrative and coordination requirements of FSCD Specialized Services are met in a timely and organized manner.
Duties and Responsibilities
Family Onboarding and Support
Serve as the first point of contact for families new to services, providing information about the agency's Specialized Services program.
Support families in completing intake paperwork and consents, and ensure documentation is collected and filed in accordance with agency and FSCD requirements.
Schedule orientation meetings and goal development sessions, ensuring attendance by required participants (e.g., family, FSCD, relevant agency staff).
Program Coordination
Submit service proposals and program plans to FSCD and ensure approvals are received in a timely manner.
Monitor timelines for plan submissions, renewals, and reporting deadlines.
Ensure the family has the most recent version of their program plan and that it aligns with FSCD requirements.
Support families in understanding FSCD service structures and navigating transitions or changes (e.g., renewals, amendments, or closures).
Liaison and Advocacy
Liaise with schools, medical teams, PDD coordinators, and other service providers to coordinate information-sharing and support a unified approach to care.
Attend school or community meetings when needed (e.g., ISPs, transition planning), to support continuity of care and clarify service boundaries.
Provide information to families about external resources and help them connect with community programs when appropriate.
Administrative Compliance
Maintain accurate electronic records in SharePoint and Jane.
Track and file signed FSCD agreements and ensure all required documents are submitted to FSCD in a timely manner.
Review and collect required reports from clinicians (without evaluating clinical content) and ensure they are submitted for FSCD reporting.
Submit service proposals, rural travel budgets, and other required documents as part of year-end or term-end packages.
Submit and follow up on signed invoices and ensure timely delivery to the finance team.
Track Specialized Services hours delivered and flag discrepancies for review.
Team Communication
Attend team meetings to share updates on administrative or funding-related requirements.
Share reminders with clinicians and team members about deadlines and documentation requirements (e.g., program plan updates, signed consents, file maintenance).
Collaborate with clinicians and leadership when families raise concerns.
Qualifications
Education
Master's Degree in Human Services, Education, Social Work, Disability Studies, or a related field.
Experience
Experience supporting families of children with disabilities.
Familiarity with FSCD, PDD, and school systems is essential.
Experience working with families navigating government or education systems preferred.
Skills and Systems
Comfortable using SharePoint, Jane, and Microsoft Office (Word, Excel, Outlook).
Strong organizational and time management skills.
Excellent interpersonal and communication skills.
Ability to maintain appropriate confidentiality and boundaries.
Other Requirements
Valid Alberta Class 5 Driver's License and reliable vehicle.
Willingness to travel within Calgary and surrounding areas.
Ability to work independently while contributing to a team-based environment.
