Seeking Physical Therapists in the Home Health setting
Our compensation model provides a guaranteed annual salary in addition to a market-leading $ / productivity point rate:
Fixed annual base salary: $70,980 / year (equivalent to a guaranteed 21 productivity points / week)
$ / Productivity Point: $65 / point (for every point earned incremental to first 21 productivity points)
As we rapidly grow our patient census and expand our clinical team, our compensation model aims to provide predictable compensation through a base salary with opportunity to earn more based on productivity.
This position is full time with the expectation of 28 productivity points completed per week. The clinician will be paid per point and has the opportunity to earn additional pay if the clinician elects to perform more than 28 productivity points per week.

Job Description:
Physical Therapist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase.
Evaluate patients and perform appropriate therapy interventions
Develop a comprehensive plan of care and coordinate multi-disciplinary care
Perform prompt clinical documentation and visit notes
Continued communication with physician, care team members, patient and family
Comfortable working in all aspects of the home health setting
Implement and maintain physical therapy protocols
Manage patient treatment and outcomes directly, or through PTA supervision
Requirements:
Current Texas Physical Therapist (PT) license
One year of physical therapy experience
Experience with Homecare Homebase is helpful, but not required
Previous Home Health experience is helpful, but not required
Current Texas driver's license
CPR certification
