Assisted Living Alberta is dedicated to Enhancing Care in the Community, helping people be healthy, well and independent in their homes and communities. Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for a Physiotherapist to join our Home Living team to provide professional support services in the homes of clients of all ages and stages of life. This position works within a collaborative multidisciplinary team consisting of various healthcare professionals from the nursing and allied health professions. This team collaborates and conducts home or site visits to deliver client-centered services to individuals with a wide variety of physical, cognitive, and mental health conditions, including complex health needs. As a PT in Southwest Home Care office, you will be providing case management as well as direct care, utilizing your strong skills in family-centered practice, coaching, assessing, organizing and communication. The Case Management duties include ongoing assessing and identifying the overall needs of the client, in collaboration with the family and other partners, and developing a care plan based on those needs. The care plan ensures appropriate services are provided and the appropriate internal and external resources are accessed. Continuing Care PTs work within a Competency Framework that includes personal and professional accountability, systems thinking, creative thinking, communicating and building relationships.

As the Physiotherapist I (PT), you will provide therapeutic interventions to enable clients to achieve optimum physical function and independence in development, disease, disability, chronic conditions, illness and functional decline. You will provide intervention (i.e. prevention, triage, screening, assessment, diagnosis, goal setting, treatment, management, education, consultation, fostering resourcefulness, case management, referral and transition support to individuals, groups, populations and communities. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. The PT collaborates with patient(s) and families in determining, implementing and evaluating appropriate rehabilitation strategies. You will participate in a variety of interprofessional teams to build awareness, enrich supports and engage in quality improvement; partnering with healthcare providers, patients, families and other stakeholders to advance services for their clients. You will contribute to quality improvement and their professional community; participates in peer mentoring; and supervises students. You may provide clinical supervision to Therapy Assistants, Therapy Aides and Provisional Physiotherapists.

Required Qualifications:
Bachelor's degree in Physiotherapy from an accredited university program. Current registration with the College of Physiotherapists of Alberta.
Additional Required Qualifications:
Proven ability to provide exceptional clinical care within a continually changing healthcare environment. Communication and assessment skills. Basic Cardiac Life Support - Healthcare Provider (BCLS HCP). The successful candidate will work independently in clients' homes, which could include congregate settings, single family dwellings, condos and/or walk-up apartments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions) as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet.
Preferred Qualifications:
Experience in Home Care. Experience with Connect Care. Excellent interpersonal and communication skills. Excellent organizational and time management skills. Demonstrated ability to work as part of an interdisciplinary team.
