Position Overview
Under the direction of the Program Manager, the Treatment Services Case Manager is responsible for the effective implementation and ongoing coordination of the case management system. This role ensures the delivery of high-quality, culturally responsive services that support clients' developmental, treatment, and psychosocial needs. The Case Manager maintains accurate and timely clinical documentation; conducts screenings and developmental assessments; participates in case reviews and conferencing; and evaluates program processes to recommend improvements.
The Treatment Services Case Manager provides direct service to clients through individual and group-based interventions, including facilitating groups, meeting with clients in the community, and delivering counselling supports. This position also provides case management services to clients on the waitlist, assists walk-in clients, and delivers single-session support in accordance with the philosophy, policies, and standards of Dilico Anishinabek Family Care.
Duties and Responsibilities
The Treatment Services Case Manager is responsible for delivering a range of clinical and supportive services aligned with the agency's mandate, including but not limited to:
Manage and coordinate the clinical waitlist, including completing required waitlist check-in calls and documentation
Provide walk-in counselling and single-session support services
Deliver ongoing case management services, including assessment, service planning, referrals, advocacy, and follow-up
Facilitate group programming and psychoeducational groups in accordance with program goals
Meet with clients in the community, homes, or other appropriate settings to support engagement and continuity of care
Conduct screenings, intakes, and developmental or psychosocial assessments
Actively participate in case reviews, case conferences, and multidisciplinary team meetings
Support clients' treatment goals through individual counselling and supportive interventions
Collaborate with internal programs, community partners, and external service providers to coordinate care
Monitor and evaluate service delivery processes and provide recommendations for program improvement
Adhere to all agency policies, procedures, and ethical guidelines
Incorporate culturally appropriate, trauma-informed, and client-centered approaches to service delivery
Perform other related duties as assigned
Qualifications and Requirements
Post-secondary degree or diploma in Social Work, Human Services, Psychology, or a related field
Relevant experience providing case management, counselling, or treatment services
Experience facilitating groups and providing individual support
Knowledge of Indigenous culture, traditions, and community-based approaches is a strong asset
Demonstrated ability to work with individuals and families experiencing complex social, emotional, and mental health challenges
Strong assessment, documentation, organizational, and communication skills
Ability to work independently and as part of a multidisciplinary team
Willingness and ability to meet with clients in community settings
Valid driver's license and access to a reliable vehicle
Satisfactory criminal records and vulnerable sector check
Hours of Work: 32.5 hours per week
