The Middlesex London Ontario Health Team (MLOHT), Middlesex London Primary Care Network (MLPCN) and Thames Valley Family Health Team (TVFHT) are seeking a Project Management. This role has been created to support a period of significant primary care transformation in Middlesex London, including the expansion of Integrated Primary Care Teams and the implementation of Supported Attachment Services. The Project Management will play a key role in moving these initiatives from design to delivery—ensuring practical implementation, clinician engagement, and measurable system impact.

Position Summary
The Project Management will lead or support the planning, implementation, and evaluation of primary care initiatives. This role requires experience in healthcare project management, quality improvement, digital health implementation, and change management, with the ability to work effectively in clinical environments and across diverse stakeholder groups.
This is a hands-on role for someone who can translate strategy into action—supporting workflow design, process mapping, digital health implementation, and continuous quality improvement using evidence-based approaches such as PDSA cycles. The successful candidate is passionate about health equity, co-design, and system improvement, and ensures that patients/clients, caregivers, clinicians, and community partners are central to planning and decision-making.
Key Responsibilities
Lead or support multiple medium-to-large scale projects across primary care and integrated care settings, aligned with MLOHT, MLPCN, and TVFHT priorities.
Apply project management, quality improvement, and change management methodologies to support Supported Attachment Services and Integrated Primary Care Team expansion initiatives.
Serve as a hands-on project lead or support, working directly with clinical and operational teams to move initiatives from planning to execution, troubleshoot barriers, and support practical implementation.
Lead or support quality improvement activities, including PDSA cycles, measurement planning, and iterative testing.
Lead or support the planning and implementation of digital health solutions in clinical environments, including workflow integration and change support.
Work with clinical and operational teams to conduct process mapping and workflow design.
Facilitate collaboration among clinicians, operational leaders, patients/clients, caregivers, and system partners.
Act as a day-to-day coordination point for internal teams and external partner organizations.
Develop and maintain project plans, timelines, risks, budgets, and communication materials.
Coordinate project meetings, working groups, and partner forums; prepare agendas, document decisions, and track action items.
Monitor progress, manage and escalate risks issues, and dependencies, and provide regular reporting to leadership and governance tables.
Support data collection, analysis, and reporting to enable evaluation and continuous improvement.
Apply a health equity lens to all project work, addressing access, inclusion, and population health priorities.
Key Competencies
Demonstrated experience managing and implementing complex projects, including digital implementation, in healthcare or clinical settings.
Understanding of primary care, integrated care models, Ontario Health Teams, Primary Care Networks, Family Health Teams, and the broader Ontario healthcare system.
Experience applying quality improvement methodologies, including PDSA and measurement for improvement.
Strong interpersonal and communication skills, with the ability to engage diverse stakeholders.
Collaborative leadership style with strong problem-solving, risk management, and decision-making skills.
Results-oriented and comfortable working in fast-paced, evolving environments.
Operates effectively in environments of ambiguity, adapting project plans, priorities, and implementation strategies in response to evolving Ontario Health and Ministry direction.
Qualifications And Experience
Post-secondary education in a related health, public administration, or business field required.
Minimum 3 years of progressive project management experience in healthcare, preferably in primary care setting.
Experience supporting or implementing digital health solutions in clinical environments preferred.
Demonstrated experience working across multiple organizations or system-level initiatives.
Hands-on experience with quality improvement and change management.
PMP certification, co-design/Experience-Based Design experience, Quality Improvement certification, Change Management certification, and strong analytical skills are assets.
Demonstrated ability to manage competing priorities and meet tight timelines.
Passion for primary care, health equity, and population health.
Working Conditions
This is a hybrid position, with a combination of remote work and in-person work. This is a new position.
