The purpose of this position is to maintain, improve, restore and optimize the health status of clients within a practice setting through direct client care for individuals, families, and communities. The Primary Health Care Nurse Practitioner functions autonomously within a collaborative practice setting while providing advanced holistic care of acute and chronic illness to clients across the lifespan. The Nurse Practitioner works as part of the primary health care team composed of physicians, nurses, medical secretaries and other health professionals.

Health PEI and the Department of Health and Wellness are further expanding the current model of primary care to a team-based collaborative care approach that will improve and sustain service delivery across the province and will bring primary care closer to where Islanders work and live.

Team-based collaborative care occurs when a number of health providers with different professional backgrounds work together with patients, families, caregivers and communities to deliver care so that the right care is provided at the right time by the right provider. Team-based care will be delivered from Patient Medical Homes in Patient Medical Neighbourhoods, a concept developed and endorsed by the College of Family Physicians of Canada (CFPC). A Patient Medical Home is a family practice defined by its patients as the place they feel most comfortable - most at home - to present and discuss their personal and family health and medical concerns

Duties will include:

  • perform comprehensive, focused health assessments including a health history and physical examination using a holistic framework and multiple assessment techniques relevant to a client's stage of development, culture and health.
  • analyze client symptoms and assessment data by applying the principles of patho physiology, pathogenesis, epidemiology, demographics and relevant research; determine the need for, order, and interpret the results of relevant screening & diagnostic tests; discuss with and involve clients in decision making regarding their health, and in taking responsibility for their health.
  • systematically compare and contrast clinical, behavioural and diagnostic data to determine differential and working diagnosis.
  • provide direct guidance and counseling on health promotion and illness prevention strategies in accordance with client health status and needs, participate in the overall evaluation of the practice setting's programs and services.
  • collaborate with interdisciplinary teams and community members to identify client population health needs and resources, and provide input as requested for community health programs.
  • ensure professional standards and clinical competence are reflected in all educational content taught by Primary Health Care Nurse Practitioners, and are demonstrated by nursing staff in the work unit and performing,
  • this position may be required to work evenings and weekends,
  • other related duties as required.

Minimum Qualifications:

  • Must have a Masters Degree in Nursing as a Nurse Practitioner or graduation from or completed the requirements to graduate from an approved and recognized Nurse Practitioner program. Extensive experience working as Registered Nurse is required.
  • Must have graduated from Family/All Ages program, preferably with a focus on Primary Care.
  • Active or eligible for licensure as a Nurse Practitioner with the College of Registered Nurses and Midwives of Prince Edward Island.
  • Critical thinking proficiency to assess, analyze, diagnose, select treatment, evaluate, and refer patients with common acute or chronic health conditions, or for health promotion / illness prevention.
  • Demonstrated ability to work effectively in an interdisciplinary environment with excellent collaborative, team building and consultative skills.
  • Excellent communication (verbal and written), facilitation and interpersonal skills in dealing with clients, families, community groups, professional colleagues, and referral partners.
  • Thorough knowledge and proficiency in medical and nursing physical assessment and treatment methods.
  • Demonstrated ability to prepare and present information on health promotion and illness prevention to the community and practice setting staff.
  • Strong analytical, problem-solving and decision-making abilities.
  • The successful applicant must meet minimal productivity requirement; demonstrated ability to be self directed.
  • Must have access to reliable transportation.
  • The successful applicant must have a good previous work and attendance record.

Other Qualifications:

  • Previous experience working as an NP in Primary Care is an asset
  • Previous experience in leadership roles would be an asset.
  • Additional relevant education and experience will be considered an asset.

External candidates may be eligible to receive a $8,000 or $18,000 Nurse Practitioner Incentive, plus a $5,000 Rural or $8,000 Student Loan Incentive depending on location of employment. To be eligible for recruitment incentives candidates must hold a 1.0 FTE, this can be a full-time role or a combination of part-time positions. Relocation Assistance maybe available to part or full-time candidates.

Salary Range:
$ $65.59 per hour (Level 20)

Bi-Weekly Hours:
75 hours Bi-weekly/Days/Monday–Friday/Evenings & Weekends as Required
