We are pleased to offer a $10,000 signing bonus and up to  $3300 in relocation assistance for this full-time permanent role at this community. About
the Role

The Licensed Practical
Nurse (LPN) reports to the Director of Care and/or designate. This position is
the role model and team leader to an assigned comunity, and responsible for coordinating
care delivery of the persons served. The LPN performs duties and monitors
departmental activities in accordance with their scope of practice, established
standards, regulations, policies and procedures to maximize individualized and
resident-centered care. 

Responsibilities

Include but not limited to:

  1. Be an engaged collaborative team player, developing and
    maintaining professional relationships that support and strengthen the
    organizational culture and purpose.  
  2. Assesses residents' needs; implements and evaluates individualized resident-centered care plans.
  3. Ensures residents rights are observed and adheres to a
    holistic philosophy of wellness.
  4. Liaises with physicians, community agency representatives, families, visitors, volunteers, other disciplines and departments to ensure quality resident-centred care is delivered.
  5. Assesses and treats all wounds on the unit and acts as
    a resource to care staff for strategies to prevent skin breakdown. Consult with
    RN or designate for changes in treatment strategies as needed.
  6. Administers prescribed medications and treatments within the limitations of the BC College of Nurses and Midwives (BCCNM).
  7. Monitors and reports changes in physical and
    psychological conditions of residents. Document changes on the resident health record,
    notifies family and physician of changes in condition.  
  8. Active participant in Interdisciplinary Care
    Conferences.
  9. Participates in care plan reviews to ensure current and individualized care plans.
  10. Supports
    the administrative requirements at the community in accordance with established
    standards, including but not limited to; admissions, discharge and ensuring
    confidentiality is maintained at all times.
  11. Actively participates in orientation of new staff, community programs, meetings, committees and educational in-services.
  12. Provides
    coaching and direction to care staff as needed and uses in the moment education
    to improve resident care which includes completing all Point of Care (POC)
    documentation. Seeks assistance/support from RN or designate as needed.
  13. Maintains
    an up-to-date knowledge of community policies and procedures, particularly
    those concerning emergency procedures including equipment management, guidance
    and assistance on safe work practices.  
  14. Performs
    other related duties as required.

Qualifications
and Skills:

  1. Graduate of an approved program for Practical Nursing
    and holds a current license with the British Columbia College of Nurses and
    Midwives (BCCNM) as an LPN.
  2. Valid Occupational First Aid Level 1 certification.
  3. Successful annual Resident Assessment Instrument (RAI)
    competency testing.
  4. Previous experience in a long term care facility an
    asset.
  5. All successful applicants must pass the vulnerable
    sector Criminal Record Check applicable to Provincial guidelines (see BCCNM
    license).
  6. Ability to read, write, and speak English proficiently
    and understand verbal and written instructions.
  7. Computer skills including the ability to navigate basic
    computer programs such as Word, Excel, Outlook.
  8. Ability to push, pull, stoop, crouch, lift and carry up
    to 25 lbs. Must be able to work with cleaners, detergents and other cleaning
    equipment.
