The Care Facilitator is responsible for assisting the Consumer/Member in the approval process for the Private Services division of the Agency. She/he is knowledgeable of Medicaid program regulations.
*Duties and Responsibilities:*
* May serve as an interpretive liaison between Third Party Assessors and Consumer/Member per Consumers/Members request.
* Perform scheduled supervisory visits to Consumer/Member, in their home, on a monthly basis per State Program guidelines.
* Assist with internal and external transfer process.
* Responsible for accurately documenting changes in Consumer/Member information.
* Produce Incident Reports when needed.
* Verify Consumer/Member eligibility and authorizations prior to admission and thereafter as directed by State Program Guidelines.
* Participate in after hours and weekend On-Call rotation.
* Prepare for State and Agency audits.
*Requirements*:
* At least a high school diploma.
* A minimum of 1 year of experience in working in an office environment, is preferred.
* Ability to provide proof of right to work in the United States.
* Proficiency in Microsoft office including but not limited to Excel and Microsoft word.
* Must have a current driver’s license and clean driving record to get on company insurance policy.
* Willing to travel as deemed necessary.
* Ability to pass a criminal background check.
Job Type: Full-time
