EMPLOYMENT OPPORTUNITY – Evenings Part time 0.70 FTE
WAGE RATE - Dependant on Qualified Experience

JOB SUMMARY:
The Care Manager is a leadership role responsible for coordinating and supervising the delivery of resident-centered care within a supportive living or long-term care environment. This position may be held by a Registered Nurse (RN) or Licensed Practical Nurse (LPN), depending on the needs and structure of the home. The Care Manager leads the nursing team in their area of responsibility, supports interdisciplinary collaboration, and ensures compliance with professional standards, organizational policies, and provincial regulations. Duties and responsibilities may include all or some of the following responsibilities depending on the size of the home and may be combined with another job title if applicable.
TYPICAL DUTIES AND RESPONSIBILITIES:
 Leadership & Team Coordination
  • Recruits, orients, supervises, and mentors nursing staff to ensure effective and consistent care delivery.
  • Provides performance evaluations, coaching, and disciplinary action as needed.
  • Acts as a role model for professionalism, empathy, and respectful communication.
  • Supports staff in advocating for resident autonomy and choice.
  •  Resident Care Oversight
  • Ensures comprehensive assessments (e.g., MDS InterRAI) are completed and used to develop individualized care plans
  • Monitors care delivery and evaluates resident responses to interventions.
  • Leads or participates in case conferences, family meetings, and interdisciplinary rounds.
  • Oversees admission, discharge, and transition processes.
  •  Clinical & Operational Management
  • Provides oversight of clinical judgment, standards of quality, and best practices.
  • Leads incident reviews, including medication and non-medication related events.
  • Manages complex resident issues and escalates to the Director of Care when appropriate.
  • Coordinates vaccine clinics, wound care programs, and other health initiatives.
  •  Quality Improvement & Compliance
  • Leads or contributes to quality improvement initiatives and audits (e.g., falls, bathing, wound
  • care).
  • Ensures compliance with CCHSS, Accreditation Standards, Accommodation Standards, and
  • internal policies.
  • Collects and reviews monthly and quarterly reports and submits to leadership.
  • Participates in site committees such as Pharmacy QI, Practice Council, and Dining Enhancement.
  •  Communication & Collaboration
  • Acts as a liaison with residents, families, physicians, Alberta Health Services, and other
  • departments.
  • Facilitates regular communication with Resident LPN Managers and interdisciplinary teams.
  • Creates and distributes memos and updates on neighborhood-specific matters.
  •  Staff Management
  • Oversees recruitment and licensing compliance for LPNs and HCAs.
  • Manages attendance, vacation approvals, and WCB-related staffing issues.
  • Promotes staff morale and provides mentorship and coaching.
  •  Coverage & Contingency Planning
  • Coordinates coverage for planned and unplanned absences.
  • Ensures clear communication of coverage plans to leadership, staff, and external partners.
  • HEALTH & SAFETY RESPONSIBILITIES:
     
  • Responsible to read, understand and comply with Park Place and provincially mandated Occupational Health and Safety policies and safe work practices and ensures mandated employee access to Worksafe BC website or manuals.
  • Responsible to educate employees annually on 'Violence in the Workplace" and "Bullying and Harrassment" in the workplace.
  • Demonstrates leadership through Involvement in all aspects of the Health and Safety Program
  • including:
    • promotes, instructs, and monitors safe work procedures
    • Promotes health and safety policy and awareness
    • Implements effective safety suggestions
    • Responsible to take every reasonable precaution to protect the safety of Residents, self, other employees, and the public.
    • Reports any near miss, injury, and accident or equipment damage to supervisor immediately and completes required reports.
    • Corrects and/or reports unsafe conditions.
    • Leads by example
    • QUALIFICATIONS AND EDUCATION:
    • Baccalaureate of Nursing or Licensed Practical Nurse designation.
    • Minimum three (3) years of recent related experience in gerontology or long-term care.
    • Current practicing registration with the applicable provincial regulatory body.
    • Experience in leadership, team coordination, and quality improvement preferred.

    • Professionalism: Demonstrates ability to effectively establish professional working relationships
    • with Residents, families, physicians, as well as both professional and non-professional members of the Care Team aiming for excellence in Residents' care.
    • Change Management: Demonstrates ability to effectively introduce and manage purposeful change that is consistent with Park Place vision, mission, and strategic directions.
    • Communication: Demonstrates ability to communicate effectively with Residents, families, the public, medical staff, and other members of the interdisciplinary team. Ability to intervene
    • effectively to resolve conflict, manage stress, and work pressure.
    • Results Oriented: Ability to apply continuous quality improvement tools and processes within a
    • dynamic environment, leading to improved care and service delivery.
    • Equipment: Demonstrates applicable computer skills and knowledge of nursing-related care equipment utilization and management.
    • EQUIPMENT USED:
      Computer, facsimile, and photocopier
      PHYSICAL DEMANDS:
    • Job requires a significant amount of lifting, bending, walking, sitting, and standing.
    • Lifting 50-lbs. maximum. More frequent lifting of 20-lbs. or less.
    • A regular day may involve four (4) hours of walking and frequent bending or crouching.
    • Must be willing to work flexible hours
