Position Clinical Pharmacist Competition Ongoing Opportunity Status Casual Department Pharmacy Union Association OPSEU Salary hour Position Summary Responsible for the provision of Clinical Pharmacy services; resolving and preventing drug related problems through medication profile review patient assessment Pharmaceutical Care Plan Development and Implementation Professional activities include provision of drug information to other health care providers including physicians nursing and other allied health professionals Participating in committees both internally and externally conducting Drug Utilization Evaluation projects and delivering in services to professional staff Basic Education and Experience Requirements Doctorate of Pharmacy degree Pharm D is preferredCertification of competency from the Pharmacy Examination Board of CanadaLicensed as a Pharmacist with the Ontario College of PharmacistsCompletion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education training and experience is preferredWorking experience in hospital setting with direct involvement in Pharmaceutical patient care preferred Strong knowledge in therapeutic drug monitoring Good understanding of clinical pharmacists role within the multidisciplinary teamIn depth understanding of use composition and clinical effects of drugsResponsibilities As a Clinical Pharmacist you will work to provide the following services to our patients Provision of pharmaceutical care to optimize drug utilization identifying potential and actual drug related problems making recommendations and monitoring for efficacy and toxicity of drugs to achieve desired patient outcomesProvision of drug information education to patients and all health care professionalsWork in collaboration with physicians and other health care providers to develop implement and monitor therapeutic plansAssist in the development of drug therapy guidelines for individual disease conditionsParticipate in medication use evaluations quality improvement initiatives and committeesProvides mentorship and supervision to pharmacy students and new staffAdditionally we would depend on your expertise to ensure that prescriptions are validated and dispensed accurately; and that medication safety concerns working with the health care team to resolve discrepancies andor problems are resolved expediently; your professional practice activities shall be documented proficientlySkills and Abilities Excellent interpersonal skills and computer skillsStrong communication skills both verbal and writtenAbility to superviseleadcoachmentorExperience working with word processing spreadsheet and database applicationsInnovative and willing to seek new challengesDemonstrated pharmacologic knowledge eg pharmacodynamics and pharmacokineticsAbility to plan implement organize problem solve and work on several projects simultaneouslySelf motivated reliable and patient focused team playerGood attendance and availability required
