Overview
Join our dedicated healthcare team as a PRN Social Worker, where you will play a vital role in supporting individuals and families through comprehensive psychosocial assessments, counseling, and case management. This position offers an exciting opportunity to make a meaningful difference in patients' lives by providing compassionate mental health and behavioral support across diverse settings. Your expertise will help empower clients to overcome challenges related to mental health, developmental disabilities, addiction, and chronic illnesses.
Duties
- Conduct thorough intake assessments, diagnostic evaluations, and psychosocial histories to determine client needs and develop personalized care plans.
- Provide individual, group, and family counseling utilizing evidence-based approaches such as cognitive behavioral therapy, psychodynamic therapy, behavioral therapy, and applied behavior analysis.
- Implement specialized interventions for populations including children with autism spectrum disorder, individuals with PTSD or Geriatrics needs, and patients requiring Alzheimer's or dementia care.
- Coordinate services through case management activities, including resource linkage, home visits, community outreach, and collaboration with medical teams to ensure continuity of care.
- Maintain accurate medical documentation in Electronic Medical Records (EMR) systems while adhering to HIPAA regulations for patient privacy and confidentiality.
- Engage in crisis intervention and crisis management to support clients experiencing acute mental health episodes or behavioral crises.
- Collect data for clinical research projects related to behavioral health outcomes, addiction counseling effectiveness, or developmental disabilities; contribute to quality improvement initiatives.
Qualifications
- Bachelor's degree in social work, psychology, counseling or related field; Master's degree preferred.
- Valid licensure or certification as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or equivalent credential.
- Proven experience working with children and families in clinical settings such as hospitals, community clinics, or home care environments.
- Familiarity with medical terminology, ICD coding (ICD-9/10), CPT coding, and medical records management.
- Knowledge of mental health disorders including PTSD, depression, anxiety disorders, addiction issues, and developmental disabilities.
- Experience utilizing EMR systems for documentation and data collection; strong organizational skills for managing patient records securely under HIPAA guidelines.
- Ability to perform diagnostic evaluations and develop treatment plans tailored to individual client needs.
- Excellent communication skills with the ability to work collaboratively within multidisciplinary teams; sensitivity when working with diverse populations including those with autism or disabilities.
