Hendricks Community Hospital Association is seeking a qualified individual to join our team

Position Highlights

Responsible to plan, organize and implement social work services for patients and families social, emotional and economic needs in relation to the impact of terminal illness. Responsible to assess needs and facilitate plans with patients/families requiring discharge planning from the hospital. The Social Worker provides services in a compassionate, culturally competent and tactful manner in order to help facilitate access to and provide continuity of care and patient advocacy. Works collaboratively with other members of the multidisciplinary team.

30 Hours a week- Day shift

Starting wage: $24.64-$30.93 an hour depending on experience

QUALIFICATIONS:

  • Mindset ready to join a hospital with the best work culture.
  • Graduate of an approved Bachelor's Degree program in social work.
  • Experience, preferred. However, new graduates welcome.
  • Knowledge regarding community support services, death, dying, grief, and bereavement.
  • Must possess verbal and written communication skills to effectively communicate with patients and family members in a caring and empathetic manner.
  • Should be able to use common sense and be a fair judge of character.
  • Good leadership, initiative, and judgment skills.
  • Computer proficiency required. Will train on EPIC software.
  • Ability to respond to critical situations in a professional and competent manner.

PHYSICAL DEMANDS AND SPECIAL CHARACTERISTICS :

  • Moderate physical work. Near visual acuity for reading and preparing reports, forms, and policies.
  • Ability to exercise independent judgment and maintain emotional stability under stress.
  • Flexibility to handle changing situations and emergency needs.
  • Ability to maintain strict confidentiality.

JOB ACCOUNTABILITY AND RESPONSIBILITY:

  • This is a multi-department position as the hospice social worker as well as the hospital discharge planner. Referrals may be ordered for resource needs in the clinic setting.
  • The Hospice Licensed Social Worker will be a core member of the hospice Interdisciplinary team who provides supportive services to help meet the hospice patient's and family's social, emotional and economic needs in relation to the impact of a terminal illness. This involves developing the plan of care, document progress notes in the EMR, complete required assessments, bereavement, coordinating grief support groups, assisting with hospice events, and annual education are expected, etc.
  • The Hospital Discharge planner will coordinate safe patient transition from hospital to home or another facility, arrange services, collaborate with doctors, nurses and insurers, lead hospital interdisciplinary meetings, facilitate the implementation and referral to community resources, etc.
