TITLE: Certified Medical Assistant (CMA)
PROGRAM: Community Based Care Coordination
JOB SUMMARY: The Certified Medical Assistant provides ancillary and supportive services within the psychiatry department.
JOB RESPONSIBILITIES AND ESSENTIAL FUNCTIONS:
Provides nonskilled nursing services that promote consumer recovery within the parameters of CMA practice. This may include obtaining and documenting a client's height, weight, blood pressure, BMI, or other symptom related communication.
Comply with all safety and health standards, rules, regulations, policies and procedures.
Participate with other health care providers in the development and modification of an Integrated Treatment Plan (ITP).
Determine and implement appropriate interventions within the client's ITP or when delegated or assigned under the supervision of a licensed provider (MD/DO, PA, APRN, RN) when acting in a clinic. Supervision may be direct (on-site) or indirect (available by phone), depending on the complexity of the task and setting.
Consults with the client and multi-disciplinary treatment team about treatment options and preferences.
Monitors progress of the person-centered or family centered ITP for all psychiatric department clients
Coordinates care (routine, urgent, crisis) among primary healthcare, behavioral healthcare, dental, pharmacy, and specialty services inside and outside the agency, including, but not limited to:
Scheduling and monitoring appointments
Scheduling follow-up services and post-hospitalization appointments
Lab work
Prescription prior authorization, appeals, adherence
Secure releases of information from past or current providers and obtains relevant information
Facilitates and monitors internal and external treatment referrals.
Monitors progress in achieving goals to demonstrate improvement in the care and clinical outcome of the consumers receiving psychiatric services.
Complete all documentation related to the position within compliance with regulatory and agency requirements.
PHYSICAL REQUIREMENTS FOR POSITION:
Must be able to move in a manner conducive to the execution of daily activities. While performing the duties of this job, the employee must communicate with others and exchange information.
The employee regularly operates equipment (listed below) on a daily basis. Occasional bending and lifting of office materials may be required.
NON-ESSENTIAL FUNCTIONS: Perform other duties as assigned by the Program Director.
EQUIPMENT USED: Including but not limited to: Telephone/cell phone, vehicle, computer, scanner, blood pressure cuff, thermometer, stethoscope, scale.
JOB QUALIFICATIONS AND REQUIREMENTS:
Certified Medical Assistant (CMA) in good standing in the state of Minnesota.
Must meet minimum qualifications as a Mental Health Practitioner as outlined in MN Statute 245i.04: Sec. 245I.04 MN Statutes
Knowledge of, and the ability to collaborate effectively with, individuals who experience chronic and severe behavioral health challenges. Knowledge of behavioral health service delivery and case management principles with a treatment service continuum.
Knowledge of Federal, State and County laws and regulations which impact upon service delivery.
Ability to work successfully independently and as part of each client's multi-disciplinary team.
Ability to work with diverse populations including age, gender, ethnicity, and cultural components of clients.
Skilled in planning, problem solving and decision-making. Must be highly organized and creative with an ability to locate resources for clients as needed.
Exceptional organization and communication skills (oral and written).
Must be able to maintain confidentiality.
Must hold a current certificate in CPR/AED/First Aid or be willing to obtain.
Must possess a vehicle valid driver's license and a willingness to travel as needed to client or organization locations throughout SE MN.
