Position Summary

We are looking for a licensed Occupational Therapist to provide high-quality, patient-centered care to individuals recovering from illness, injury, and functional limitations. In this role, you will conduct evaluations, develop treatment plans, assess home and work environments, and support patients in regaining independence and improving daily living skills.

The ideal candidate demonstrates strong clinical competency, compassion, professionalism, and a genuine desire to help patients thrive in their home environment.

Key Responsibilities

  • Perform physical, cognitive, and functional assessments to develop individualized treatment plans.
  • Evaluate home and work environments and recommend modifications to enhance safety and independence.
  • Provide education and hands-on training to patients, caregivers, and family members.
  • Develop and implement rehabilitation programs to help patients regain mobility, coordination, and daily living skills.
  • Recommend and train patients in the use of adaptive or assistive equipment.
  • Prepare patients for a safe return to personal routines, work, or school when appropriate.
  • Track patient progress, document services, and update care plans in accordance with agency standards.
  • Maintain compliance with all federal, state, and local regulatory requirements.
  • Stay informed on current therapy practices and participate in ongoing professional development.

Requirements

  • Bachelor's or Master's Degree in Occupational Therapy from an ACOTE- or AOTA-accredited program
  • Active Florida OT license
  • National Board for Certification in Occupational Therapy (NBCOT) certification
  • Up-to-date knowledge of OT treatment practices, rehabilitation techniques, and clinical documentation
  • Strong communication, interpersonal, and problem-solving skills
  • Ability to provide compassionate care to patients with diverse needs and personalities
  • Comfortable using EMR systems and basic technology for documentation.
  • Experience with Alora Plus EMR preferred.
  • Reliable transportation for home visits (home health roles only)
