• Provide high-quality occupational therapy services to residents in our skilled nursing facility
• Perform evaluations and develop individualized plans of care focused on increasing independence with ADLs, functional tasks, and overall quality of life
• Implement evidence-based interventions for bathing, dressing, grooming, toileting, feeding, cognitive/perceptual skills, fine motor coordination, and functional mobility as it relates to daily tasks
• Collaborate with an interdisciplinary team (nursing, PT, SLP, social work, activities staff, families) to ensure coordinated, resident-centered outcomes
• Fabricate or recommend adaptive equipment, DME, and environmental modifications to enhance independence and safety
• Monitor, document, and report progress toward established goals in compliance with facility, state, and regulatory standards
• Support restorative and maintenance programs and provide education to residents, staff, and families regarding safe techniques and task participation
• Foster a positive, encouraging environment that promotes dignity, function, and optimal independence

• Current Occupational Therapist (OT) license in the state of WV or eligibility for licensure
• Graduate of an accredited Occupational Therapy program
• Passionate about helping residents achieve their highest level of functional independence in a long-term care and rehab setting
