Job Summary
Job Summary: Job incumbent will provide and coordinate outreach services to the senior citizens (those 65 years and older) of the TCC service region. This position will work closely with elders, community and volunteers, requiring extensive travel and providing services throughout the TCC region. The Elder Navigator will conduct a comprehensive, systemic assessment and analysis of all available data and information concerning to identify needs of senior citizens in the TCC region. This position will support elderly access to the resources and facilitate communication between the region’s senior citizen community and TCC.
Adhere to the TCC Ch’eghwtsen’ model of service and guiding principle which requires providing timely and effective service along with the ability to interact with others in a way that inspires trust and demonstrates respect, compassion and empathy.
Essential Functions

Representative Duties: Under the general supervision of the Education & Indigenous Learning Manager, job incumbent will:
1. Be responsible for coordinating and implementing wellness and prevention education, increase referrals, and identify persons in need, and coordination with youth programs as necessary, and for coordinating with partners such as Denakkanaaga, FNA and others.
2. Assist in the gathering of the required data as it relates to the needs of the elderly community. Maintain compliance with data collection requirements. Complete and submit routine paperwork in a timely manner.
3. Coordinate with the various TCC departments to advocate, connect, mentor, and support needs of the elderly. Identify and provide outreach services within the senior citizen community.
4. Developing a strong working relationship with tribal leadership, Adult Protection Services, law enforcement, and tribal health service systems.
5. Complete reports and documentation in a timely manner as required by the program supervisor, funding sources, and agency policy. Document all client services in accordance with program procedures. Keeps client documentation and reports current. Maintain a daily log of activities and services provided.
6. Travel to remote communities to assist in coordination of services for the elderly.
7. Coordinate educational programs for the elderly about relevant issues or topics such as technology, health, safety, or environmental issues.
8. Maintain knowledge of community resources, making appropriate community referrals, and follow up procedures.
Other Responsibilities:
1. Perform other duties as assigned.
2. May work some weekends and after business hours on a flexible, but punctual, schedule.
Minimum Qualifications
Minimum Qualifications:
1. Bachelor's degree in the field of Psychology, Counseling, Social Work or related Human Services; emphasis in the practice of Behavioral Health preferred. Candidate may have a combination of work and educational experience to meet general degree requirement.
2. One-year experience in Human Services or Health preferred.
3. Must pass background check pursuant to federal Indian Child Protection and Family Violence Prevention Act requirements and State of Alaska background check.

1. Strong understanding and sensitivity to Alaska Native culture and lifestyle, with demonstrated experience working effectively with Alaska Native population.
2. Demonstrated ability in work ethic, integrity, and positive influence in the community.
3. Exceptional customer service skills.
4. Strong written, analytical, verbal, and interpersonal skills.
5. Ability to keep accurate records and produce timely and accurate written reports.
6. Ability to maintain strict confidentiality as required for HIPAA, & other regulatory requirements.
7. Ability to learn and apply program/agency operating policies and procedures.
8. Ability to manage multiple priorities and tasks concurrently and meet deadlines.
9. Ability to establish and maintain cooperative working relationships, including outside contacts.
10. Ability to work independently as well as with teams.
Supervision: This position has no supervisory responsibilities.
Supervision
Physical demands: There are no unusual demands of the position. Requirements are those of normal office type work with extended hours of sitting and operation of standard office and computer equipment. Job incumbent may be required at times to lift and carry up to fifty pounds in support of material. Long work hours may be required during various times throughout the year as needed and required. Must be able to work in a busy, sometimes stressful environment.
