DutiesThe Clinical Pharmacist duties include, but are not limited to:
- Reviews prescription orders electronically entered by providers in the context of the patient's existing medication profile.
- Communicates with providers as necessary to clarify prescription orders, particularly with regard to:
- Critical drug interactions
- Duplicate drug therapy
- Inappropriate or confusing dosing, directions or quantities
- Non-formulary and restricted drugs.
- Verifies and electronically finishes pending prescriptions entered in CPRS.
- Manually enters handwritten prescriptions when appropriate.
- Removes or clarifies unapproved abbreviations before the prescription is dispensed to the patient.
- Removes or clarifies confusing/inappropriate provider comments before the prescription is dispensed to the patient.
- Assists in the enforcement of Pharmacy and Medical Center Policies regarding non-formulary and restricted drugs.
- Flags restricted drug/non-formulary drug orders in VISTA and enters a note in the patient's chart in CPRS identifying both the resident and the attending as additional signers informing of the drug status and the directions for obtaining approval for use.
- Checks prescriptions filled by Pharmacy Technicians and students for completeness and correctness prior to the order being dispensed or mailed to patients.
- Answers questions regarding Pharmacy policy or law, drug information, or prescription status from patients, providers and other staff.
- Counsels and educates patients and/or caregivers about prescribed drug therapy.
- Assists in or supervises the reconstitution of medications for prescriptions as necessary.
- Assists in the reporting and electronic documentation of allergies and adverse drug reactions.
- Documents interventions in a pharmacy note in CPRS when appropriate.
- Works at the outpatient pharmacy in-take window/booth to counsel patients, process newly entered prescriptions as requested by patients, process prescription refills as requested by patients, process prescription orders in the pending file as time permits and assists patients with all outpatient related issues.
- Assists in the development and implementation of Medical Center and Pharmacy Service policies and procedures regarding safety and quality management.
- Works in the discharge section of the pharmacy to process medications for patients being discharged from the hospital/Cares Tower/Domiciliary, patients going on pass from hospital/Cares Tower/Domiciliary, and for patients being transferred to nursing homes (when assigned to Wade Park Outpatient Pharmacy).
- Performs medication reconciliation and other clinical duties when assigned to the discharge process at Wade Park.
- Works with other clinical pharmacists to provide comprehensive clinical, informational and educational services.
- Reviews the computerized medication profiles, communicates with providers to resolve problems and performs medication reconciliation for patients.
- Plans and provides clinical pharmacy services on assigned inpatient wards/units when assigned to Wade Park Outpatient Pharmacy.
- Assists in the management of cost-containment programs and inventory management per Pharmacy Service policy and communications including:
- 90-day prescription fills
- Tablet splitting
- Drug conversions
- Drug shortages/recalls
- Prescription shortages
- Serves as a legally responsible Pharmacist with regard to the stocking and dispensing of controlled substances by the Outpatient Pharmacy in accordance with applicable federal drug laws and VA directives.
- Communicates stock shortages to the procurement technician.
VA Careers - Pharmacy:
Work Schedule: Monday - Friday rotating 8:00am to 4:30pm 9:30am to 6:00pm, including holidays and weekends.
