PURPOSE OF JOB: Ohio County Healthcare exists to help make a difference by caring, serving and educating

individuals relative to their specific health needs. Through these actions the general health of Ohio County and

surrounding areas will improve.

MISSION STATEMENT: The Rehabilitation Services Department provides the highest quality standard of Physical

Therapy, Occupational Therapy, Speech Therapy, Pulmonary Rehab, Cardiac Rehab, Diabetes Education, Athletic

Training Services, and etc. to our community. Optimum care will be given without regard to age, race, color,

religion, gender, sexual preference, or national origin in a cost-effective environment.

WORKING ENVIRONMENT: Usual working space is a well-ventilated area. Requires standing, walking, turning,

bending, stooping, and lifting patients. Fine and gross motor skills. Environment may be stressful at times,

therefore ability to work under stressful conditions is necessary. Also always subject to bloodborne pathogens

in patient care. May have occasional exposure to radiation during radiology procedures and to dangerous

chemicals (cleaning and disinfecting agents).

ESSENTIAL JOB FUNCTIONS:

1. The supervisor will be responsible for covering all manager duties when the Director is not available/on

premises. a. The supervisor will also work with the Director to complete all duties in a timely manner. b. Must be capable of identifying situations that may adversely affect patient outcomes, compliance, and reimbursement. c. Have working knowledge of payors and their impact on Out-Patient Rehabilitation Clinics. d. The Supervisor will follow department policies and Director recommendations to correct problems in these areas. e. If guidance is needed, will seek guidance from the Director of Rehabilitation Services. f. Will perform daily tasks/duties as requested from the Director.

2. The Physical Therapist will provide evaluations to patients demonstrating accurate and timely documentation of diagnosis, history, objective measurements, special testing, establishing a need for skilled physical therapy and a plan of care. a. They will perform treatments assuring accurate documentation of all components of a SOAP note including billing with time and units with supporting documentation. b. They will perform re-certifications every 30 days for out-patients including updating of goals and modifications in frequency/duration. c. They will perform 10th visit/progress notes as needed meeting Medicare guidelines. d. They will complete discharges for patients within 30 days of expiring POC. e. They will utilize information targeting insurance allotment of visits, track visits and submits for insurance authorization in a timely manner. f. They will serve as supervising therapists to support staff including: Physical Therapy Assistants and Rehab Techs. g. Physical Therapists will also be competent in the areas/modalities of treatment that they provide.

3. Is responsible for providing supervisory functions for site of Rehabilitation Operations in doing the following tasks: a. Maintains the work schedules, assigns duties assuring work and schedules are distributed and assures optimal staffing to provide patient care. b. Assist with addressing staff concerns. c. Assures daily assignments/tasks are being accomplished and staff remain productive and efficient. d. Assist with development and adherence to all disciplines specific evidence based and standards of practice within the department. e. Ensure therapy service provision and documentation of services are compliant according to departmental guidelines/regulations. f. Assist with competency checklist completion. g. Provide input on annual performance appraisals. h. Works in collaboration with the director to acquire additional staffing as needed and assuring new hires are appropriately trained/serve as preceptor i. Collaborates with other departments to meet staff, clinic and organizational needs. j. Orders and receives supplies, works with outside vendors and contract groups to assure clinic needs are met. k. Performs quality assurance audits and patient satisfaction surveys routinely. Guiding staff in needed improvements. l. Maintains time and attendance logs, keeping Director aware of excessive occurrences that need attention. m. Collaborates with Director in poor employee performance matters. n. Is a first contact for customer service issues/address customer concerns. Is the clinic's customer service representative for patients, providers and other hospital departments. Resolving and addressing issues as they may arise. o. Utilizes Microsoft Office (Word, Excel, Outlook) p. Proficient in Meditech EMR q. Utilizes ordering programs for supplies management, as needed r. Other duties as needed within support services s. Demonstrate good communication with upper management. t. Assist with strategic planning, development and implementation of new service lines and service improvements. u. Attend leadership training/monthly meetings as determined by manager/director. v. Be an example of leadership during daily therapist's tasks. w. Be familiar with disciplines within your clinic to assist with clinical leadership and staff support. x. Be a point of contact and support for your therapists. y. Ensure positive morale within the department. z. Works 60% of the time providing discipline specific treatments to patients and 40% of time in supervisory work, this may fluctuate at times due to clinic needs. aa. Participate in other duties as assigned by manager/director.

12. Shows evidence of prompt and thorough response in handling correspondence, reports and special projects as assigned.

B. Maintains safe and therapeutic environment.

1. Takes necessary safety precautions.

2. Reports any faulty equipment, infection or safety hazards to appropriate personnel and understands completion of event report.

3. Uses equipment and supplies in safe, efficient manner. Charge capture methods utilized according to established process.

4. Knows and responds appropriately to all emergency codes. Reports to assigned area during emergency event and carries out assigned functions in timely manner.

5. Familiar with waste segregation policy.

6. Knows and utilizes proper body mechanics.

7. Accountable for ensuring patient/staff safety during the course of performing duties.

8. Makes appropriate request for supplies or equipment needed.

C. Upholds standards of practice and code of ethics for all age groups.

1. Performs all procedures according to established policies and procedures of OCH and adheres to legal compliance policies and other regulatory issues.

2. Recognizes and supports patient rights. Maintains confidentiality in all hospital matters. Adheres to HIPAA policies and Code of Ethics

3. Adheres to bloodborne pathogens and infection control/prevention policies including the exposure control plan and employee health protocols.

4. Follows and upholds hospital personnel policies.

5. Committed to continuous performance improvement/quality improvement and actively participates and supports the organization's improvement efforts.

6. May be asked to be a member of QI Team

7. Actively participates in review and revision of policies and procedures as well as in development of new policies.

8. Demonstrates understanding of computer resources such as Policy Tech, Common Share, Paycom, etc.

9. Dress and grooming reflect pride in self and the hospital. Adheres to the departmental as well as hospital wide dress code.

10. Courteous friendly and helpful to new employees, students and personnel from other areas. Explains work accurately, clearly and according to established policy and procedure.

11. Maintains required hours for licensure.

12. Attends staff meetings 80% per year.

13. Attends mandatory education offerings including annual hospital education.

14. Attends 50% of in-services per year.

15. Dependable and reliable in attendance and punctuality. a. Perfect attendance – exceeds standard. b. Absent 1-5 occurrences – meets requirement. c. Absent 6 or more occurrences – needs improvement, does not meet standard.

Qualifications

QUALIFICATIONS:

1. EDUCATION: Masters or Doctorate of Physical Therapy

2. EXPERIENCE:

A. REQUIRED: Experience of 1-2 years of Physical Therapy and interest in leadership development.

B. PREFERRED: Experience of 3-5 years in Physical Therapy with 2-3 years of leadership experience.

3. OTHER REQUIREMENTS: BLS certification or obtain within six months from hire. Keyboarding skills,

self-motivated, excellent interpersonal and communication skills.
