Required Qualifications
Minimum requirements include the following:
v High school diploma or equivalent required.
v Valid Texas driver's license with a clean driving record.
v Certified Nursing Assistant (CNA) certification preferred.
v Previous experience working in a long-term care or healthcare setting preferred.
v Excellent communication and interpersonal skills.
v Ability to work effectively both independently and as part of a team.
v Strong organizational skills and attention to detail.
v Compassionate and patient-centered approach to resident care.
v Ability to maintain confidentiality and professionalism at all times.
Major Duties and Responsibilities
v Safely transport residents to and from medical appointments, therapy sessions, and recreational outings in accordance with established schedules and routes.
v Ensure the comfort and safety of residents during transportation, including assisting with boarding and disembarking from the vehicle and securing mobility aids as needed.
v Communicate effectively with residents, family members, and staff to coordinate transportation logistics and address any special needs or preferences.
v Maintain the cleanliness and safety of the facility's van, including regular inspections, cleaning, and maintenance tasks.
v Adhere to all traffic laws, regulations, and facility policies and procedures to ensure the safety of passengers and compliance with transportation standards.
v Provide assistance to residents as needed, including escorting them to and from the vehicle and assisting with mobility devices and personal belongings.
v Document transportation activities, mileage, and any incidents or concerns according to established procedures.
v Collaborate with the interdisciplinary team to ensure the overall well-being and satisfaction of residents and contribute to a positive and supportive care environment.

v Treats all residents with dignity and respect. Promotes and protects all residents' rights.
v Establishes a culture of compliance by adhering to all facility policies and procedures. Complies with standards of business conduct, and state/federal regulations and guidelines.
v Follows appropriate safety and hygiene measures at all times to protect residents and themselves.
v Maintains confidentiality of protected health information, including verbal, written, and electronic communications.
v Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel. Reports any retaliation or discrimination to HR or compliance officer.
v Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor and/or administrator. Protects residents from abuse, and cooperates with all investigations.
v Reports any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy.
v Coordinates all life safety and emergency drills and trainings. Fulfills responsibilities as assigned during implementation or activation of the facility's emergency plan.
v Delegates administrative authority, responsibility, and accountability to other maintenance personnel as deemed necessary to perform their assigned duties.
v Performs and monitors required inspections of facility equipment.
v Reports work-related injuries and illnesses immediately to supervisor.
v Follows established infection control policies and procedures.
v As a condition of employment, completes all assigned training and skills competency.
v Maintains a safe and secure working environment free of objects/situations that could cause harm and/or falls to residents, families, visitors, and associates.
v Performs routine repair and maintenance to facility.
v Performs regular safety inspections of the facility and documents the findings.
v Submits accident/incident reports within twenty-four (24) hours after their occurrence.
v Participates in surveys/inspections by external agencies.
v Develops a plan of correction for maintenance deficiencies identified during a facility survey.

v Meets general health requirements according to facility policy, including medical and physical exams and checking immunity status to various infectious diseases.
v Ability to work beyond normal working hours and on weekends and holidays when necessary. May be subject to call back for emergencies.
v Ability to assist in evacuation of residents during emergency situations.
v Ability to bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position.
v May be subject to falls, burns from equipment, and/or odors throughout the day; encounter reactions from dust, tobacco smoke, disinfectants, and other air contaminants.
v Subject to exposure to infectious waste, diseases and/or conditions which include AIDS, Coronavirus, Hepatitis B, and Tuberculosis.
v May be subject to hostile or emotional residents, family members, visitors or personnel.
v Ability to read and interpret blueprints and building codes.
v Ability to observe, listen and communicate effectively.
v Proficiency in computer skills and working knowledge of current PC applications, including but not limited to Microsoft Office and Google Chrome.
v Ability to cope with the mental, emotional and physical stress of the position.
v Ability to tolerate varying temperatures in hot/cold and/or wet environments.
v Ability to lift, move, push or pull a minimum of 35 pounds to a minimum of 5 feet and be able to push, pull, move and/or carry such weight a minimum distance of 50 feet.
v Works in all areas throughout the facility.
