Physical Therapist

Job Summary
We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will be responsible for evaluating, planning, and implementing individualized treatment programs for patients with various physical impairments. This role requires a comprehensive understanding of anatomy, physiology, and medical terminology, along with the ability to work collaboratively in both inpatient and outpatient settings. The Physical Therapist will focus on enhancing patient care through effective discharge planning, care plans, and rehabilitation strategies.

Responsibilities

• Conduct thorough assessments of patients to develop tailored physiotherapy treatment plans.
• Implement therapeutic interventions including trigger point therapy, orthopedics, and geriatrics-focused care.
• Collaborate with multidisciplinary teams to ensure comprehensive patient management in acute care and home health settings.
• Maintain accurate patient records using EMR systems and document progress towards rehabilitation goals.
• Educate patients and families on treatment plans, exercises, and home care strategies to promote recovery.
• Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
• Provide support in discharge planning to facilitate smooth transitions from inpatient or outpatient care to home or community settings.

Requirements

• Valid license to practice as a Physical Therapist in the relevant state.
• Strong knowledge of anatomy, physiology, medical terminology, and physiotherapy techniques.
• Experience in various settings including inpatient hospitals, outpatient clinics, and home health care is preferred.
• Familiarity with EMR systems for effective documentation and patient management.
• Excellent communication skills with the ability to educate patients effectively.
• Demonstrated ability to work collaboratively within a healthcare team while providing exceptional patient care.

Pay: $ $95.00 per hour
