Overview
We are seeking a dedicated and compassionate Enhanced Care Management (ECM) Specialist to join our team. The ideal candidate will play a vital role in coordinating comprehensive care for individuals with complex health and social needs. This position offers an opportunity to make a meaningful impact by providing personalized support, crisis intervention, and resource navigation to diverse populations, including those involved in child welfare, Medicaid, and workforce development programs. The ECM Specialist will work collaboratively with healthcare providers, social service agencies, and community organizations to promote holistic well-being and improved health outcomes.
Duties
- Conduct thorough assessments to identify clients' medical, behavioral health, social, and environmental needs.
- Develop individualized care plans that address physical health, mental health, addiction recovery, child welfare, and social determinants of health.
- Provide crisis intervention and crisis management support to clients experiencing urgent or emergent situations.
- Utilize motivational interviewing techniques to encourage behavioral change and engagement in treatment plans.
- Coordinate services related to behavioral health, addiction counseling, Section 8 housing assistance, and Medicare programs.
- Supervise or mentor junior staff or interns as needed to ensure quality service delivery.
- Collaborate with healthcare providers, social workers, child welfare agencies, and workforce development programs to facilitate comprehensive care.
- Maintain detailed documentation of client interactions, progress notes, and service plans in compliance with organizational policies.
- Support clients in navigating community resources such as childcare services, housing assistance, and employment programs.
- Participate in ongoing training related to crisis intervention, behavioral health best practices, and social work methodologies.
Skills
- Strong background in crisis intervention and crisis management strategies.
- Experience or knowledge in addiction counseling and behavioral health services.
- Proficiency in social work principles with an emphasis on client-centered approaches like motivational interviewing.
- Familiarity with child welfare systems, Section 8 housing programs, Medicare policies, and workforce development initiatives.
- Supervisory or mentoring experience is preferred to guide team members effectively.
- Excellent communication skills for engaging diverse populations including children and families.
- Ability to assess complex situations quickly and develop appropriate intervention plans.
- Compassionate attitude with a commitment to improving client outcomes through holistic care coordination. This role is integral to delivering high-quality care management services that empower individuals toward healthier lives while fostering collaboration across multiple community resources.
Job Types: Full-time, Contract
Pay: $25.00 per hour
