Job Summary
The Housing Supports Specialist provides direct services to Medicaid-eligible individuals in need of housing stabilization. This role assists members with identifying housing options, completing applications, navigating landlord relationships, connecting to benefits, and sustaining housing through ongoing support and advocacy.

Responsibilities

  • Conduct initial housing needs assessments, develop individualized housing support plans, and update plans as needs change.
  • Assist members with searching and applying for housing, including completing rental applications, gathering documentation, and attending housing appointments.
  • Coordinate move-in supports, such as budgeting, securing furnishings, and household setup.
  • Provide tenancy-sustaining services, including landlord mediation, coaching on lease compliance, and connection to community supports.
  • Support members in accessing income supports, transportation, mental health services, employment programs, and benefits.
  • Maintain timely documentation in accordance with Medicaid requirements, including service notes, assessments, and progress tracking.
  • Participate in case conferences, community meetings, and landlord engagement activities.
  • Uphold trauma-informed, strengths-based, and equitable care practices.

Requirements

  • Bachelor's degree in Social Work, Human Services, Counseling, Public Health, or related field; equivalent experience considered.
  • 1–3 years experience serving vulnerable populations (homelessness, behavioral health, medical case management, outreach, or similar).
  • Strong knowledge of NJ community resources, MCO processes, and local housing systems (preferred but not required).
  • Valid driver's license and reliable transportation for field work.
  • Ability to pass state-required background checks.
  • Excellent communication skills with the ability to advocate effectively for clients.
  • Ability to handle sensitive situations with professionalism and empathy.
  • Relevant experience in social services or housing programs is a plus.
  • Must demonstrate strong organizational skills and attention to detail to manage multiple cases efficiently.

Preferred Qualifications

  • Familiarity with New Jersey MCO Housing Supports, Continuum of Care, HMIS, and Fair Housing laws.
  • Experience working with individuals facing substance use, mental health conditions, justice involvement, or chronic homelessness.
  • Certifications such as CPR, CHW, CPRP, or Housing Specialist credentials (a plus).

  • Strong communication, motivational interviewing, and problem-solving skills.
  • Ability to manage a caseload of 10–15 members while balancing field work and documentation.
  • Cultural humility, empathy, and commitment to equitable service delivery.
  • Comfortable traveling, meeting members in the community, and working independently.
