Job Summary
We are seeking a compassionate and dedicated Case Manager to join our team in providing comprehensive support to individuals facing various challenges. The ideal candidate will have a strong ability to be self motivated, compassionate and dedicated with a focus on empowering clients through personalized care plans. This role involves coordinating services, advocating for clients, and ensuring access to necessary resources. The position offers an opportunity to make a meaningful difference in the community by assisting clients in overcoming obstacles and achieving stability.
Responsibilities
- Conduct thorough assessments of clients' needs goals and barriers.
- Develop individualized care plans
- Provide support to clients during critical moments. Connection, accountability and compassion driven communications.
- Coordinate with community resources including low barrier housing and employment in our community.
- Dependable during work hours for reliability
- Monitor client progress regularly and adjust care plans accordingly to ensure goals are met.
- Advocate on behalf of clients within healthcare systems, social services, and legal agencies to secure necessary services.
- Maintain detailed documentation of client interactions, service plans, and progress reports in compliance with organizational policies.
Requirements
- Experience in crisis intervention, crisis management, or behavioral health settings.
- Strong background in social work or related fields is highly desirable.
- Excellent communication skills with the ability to motivate and empower clients. While also being able to successfully communicate with other professionals.
- Ability to Guild- clients through complex service systems.
- Ability to handle sensitive situations with professionalism and compassion while maintaining confidentiality.
- This position is vital in supporting vulnerable populations by providing tailored case management services that foster independence and well-being within the community.
- Ability to document accurately and promptly, as well as ask for help when needed.
- Communications with team and ability to multi-task, driven to share motivation.
Pay: $ $27.00 per hour
Expected hours: 32.0 – 38.0 per week
