Job Summary
We are seeking a compassionate and dedicated Case Manager to join our team in providing comprehensive support to individuals facing various challenges. The ideal candidate will have a strong ability to be self motivated, compassionate and dedicated with a focus on empowering clients through personalized care plans. This role involves coordinating services, advocating for clients, and ensuring access to necessary resources. The position offers an opportunity to make a meaningful difference in the community by assisting clients in overcoming obstacles and achieving stability.

Responsibilities

  • Conduct thorough assessments of clients' needs goals and barriers.
  • Develop individualized care plans
  • Provide support to clients during critical moments. Connection, accountability and compassion driven communications.
  • Coordinate with community resources including low barrier housing and employment in our community.
  • Dependable during work hours for reliability
  • Monitor client progress regularly and adjust care plans accordingly to ensure goals are met.
  • Advocate on behalf of clients within healthcare systems, social services, and legal agencies to secure necessary services.
  • Maintain detailed documentation of client interactions, service plans, and progress reports in compliance with organizational policies.

Requirements

  • Experience in crisis intervention, crisis management, or behavioral health settings.
  • Strong background in social work or related fields is highly desirable.
  • Excellent communication skills with the ability to motivate and empower clients. While also being able to successfully communicate with other professionals.
  • Ability to Guild- clients through complex service systems.
  • Ability to handle sensitive situations with professionalism and compassion while maintaining confidentiality.
  • This position is vital in supporting vulnerable populations by providing tailored case management services that foster independence and well-being within the community.
  • Ability to document accurately and promptly, as well as ask for help when needed.
  • Communications with team and ability to multi-task, driven to share motivation.

Pay: $ $27.00 per hour

Expected hours: 32.0 – 38.0 per week
