Position Overview

As an Occupational Therapist, you will assess and treat residents to improve function, independence, and overall quality of life. This role includes developing evidence-based treatment plans, delivering skilled therapy services, and collaborating closely with the interdisciplinary team.

Key Responsibilities

  • Evaluate residents to determine functional impairments and therapy needs
  • Develop and implement individualized treatment plans focused on ADLs and IADLs
  • Track progress and modify treatment plans as needed in coordination with the IDT
  • Recommend adaptive equipment and home modifications to promote independence
  • Educate patients, caregivers, and families on therapeutic strategies
  • Ensure accurate, timely documentation per state, federal, and facility guidelines
  • Participate in team meetings and care planning activities
  • Adhere to all regulatory and facility policies

Qualifications

  • Active Texas Occupational Therapist license
  • In good standing with all applicable licensing boards
  • SNF or long-term care experience preferred
  • Familiarity with Net Health is a plus
  • Strong communication and interpersonal skills
  • Houston, Texas, United States
