Clinical Director

Job Overview
We are seeking a dynamic and experienced Clinical Director - Home Health RN to lead and oversee our home health care services. This pivotal role involves managing clinical operations, supervising nursing staff, and ensuring the delivery of exceptional patient care in a home-based setting. The ideal candidate will possess a passion for patient-centered care, strong leadership skills, and extensive knowledge of home health practices, including hospice care, assisted living, and acute care environments. As Clinical Director, you will drive quality improvement initiatives, ensure compliance with healthcare regulations such as HIPAA and Medicare standards, and foster a collaborative team culture that prioritizes patient safety and satisfaction.

Responsibilities

• Lead and coordinate all clinical activities within the home health agency, ensuring high standards of patient care and safety.
• Supervise nursing staff, including nurse management, shift management, and performance evaluations to promote professional growth and accountability.
• Develop, review, and update individualized care plans for residents' , ensuring they meet medical needs and regulatory requirements.
• Monitor patient progress through diligent case management, patient monitoring, and documentation review to optimize health outcomes.
• Oversee medical records management, ensuring accurate documentation in EMR systems while maintaining strict adherence to HIPAA privacy regulations.
• Collaborate with interdisciplinary teams to coordinate hospice care, assisted living services, and other specialized home health programs.
• Manage operational aspects such as budgeting, resource allocation, and compliance with Medicare guidelines to sustain efficient service delivery.
• Conduct regular audits of medical terminology usage, physiology and anatomy knowledge application, and documentation accuracy to uphold clinical excellence.

Experience

• Demonstrated supervisory experience in home health or hospice care environments with a track record of effective nurse management.
• Extensive knowledge of medical terminology, physiology, anatomy, and the regulatory landscape including HIPAA compliance.
• Familiarity with EMR systems for documentation review, patient monitoring records, and medical records management.
• Strong leadership skills with the ability to motivate teams while managing shift schedules efficiently.
• Experience in case management strategies that improve resident outcomes within assisted living or hospice settings is preferred.
• Knowledge of Medicare regulations related to home health services is essential for maintaining compliance and funding.

Job Type: Full-time

Pay: $ $65.00 per hour

Expected hours: 40 per week
