Age at Home is seeking a Client Liaison to join our team. This full-time, benefited role is ideal for a professional who excels at client satisfaction, care coordination, and business development. You'll serve as the primary point of contact for clients, families, referral partners, and prospective clients—ensuring excellent service while supporting agency growth.

What You'll Do Client Care & Service Oversight
- Serve as the primary liaison between clients, families, caregivers, and the agency
- Schedule and conduct required six-month in-home care plan reviews
- Review, update, and document care plans in compliance with regulations
- Communicate care plan changes to caregivers and client designees
- Identify, problem-solve, and resolve client care concerns
- Respond to family concerns during weekday business hours
- Introduce caregivers to their new client
- Document all client, family, and caregiver communications
- Collaborate closely with office staff using a "Close the Loop" approach
Sales & Business Development
- Manage and respond promptly to new client inquiries and referrals
- Conduct in-home and virtual assessments with a focus on converting inquiries into services
- Educate families on Age at Home services and care options
- Build strong relationships with referral partners (hospitals, rehab facilities, social workers, elder law attorneys, and community organizations)
- Participate in networking events and community outreach
- Track leads, referrals, and outcomes in agency systems
- Collaborate with leadership to meet or exceed growth and admissions goals
Facility Admission & Discharge Coordination
- Communicate with discharge planners during facility stays
- Attend discharge meetings when requested
- Follow up pre- and post-discharge to support continuity of care
Schedule & Travel
- Typical hours: Monday–Friday, 8:30am–4:30pm
- Participation in a periodic on-call rotation
- Paid travel time and mileage reimbursement (per company policy and IRS standards)
What We're Looking For
- High school diploma or equivalent
- Valid driver's license, reliable vehicle, and current auto insurance (100/300K)
- Strong communication, organization, and time management skills
- Background in healthcare, human services, sales, or community outreach preferred
- Proven ability to build relationships and close service admissions
- Self-motivated and comfortable managing an independent schedule
- Comfortable using smartphones, computers, and office software
- Professional appearance and adherence to dress code
