Greenfield Rehabilitation Agency is excited to announce an opportunity for an Occupational Therapist (OTR) to join our therapy team at our newly acquired senior community in Sauk City, WI.

This role offers flexibility for full-time, part-time, or PRN schedules—a great opportunity for a dedicated and compassionate clinician to make a meaningful difference in the lives of residents while working in a supportive, therapist-led environment.

Your Responsibilities

  • Deliver compassionate and effective occupational therapy services

  • Develop and implement individualized treatment plans to improve functional independence and quality of life

  • Support residents in achieving personal goals and progress milestones

  • Accurately document care and maintain compliance standards (experience with Net Health a plus)

  • Collaborate with the therapy and nursing teams to ensure patient-centered outcomes

  • Educate residents and families on safety and daily living strategies

  • Participate in team meetings and contribute to care planning

What You Bring

  • Current Wisconsin OTR license

  • Strong clinical skills and a genuine passion for geriatric care

  • Excellent communication and teamwork abilities

Qualifications
  • Graduate of an accredited school with a bachelors, master's, or doctoral degree in Occupational Therapy.
  • License as required by the State of Wisconsin to practice as an Occupational Therapist.
