Job Title: BHCC/ECM Case Manager
Department: ECM
FLSA Status: Non-Exempt
Reports To: Health Center Manager
Hours: Tuesday - Friday 7:30am to 6:00pm
Location: Redding
POSITION SUMMARY:
Under general supervision, the BHCC/ECM Case Manager is responsible for engaging patients and assisting with access to community resources. The BHCC/ECM Case Manager will assist providers with complex patients and offer general support when situations arise. This position will require both medical floor and field case management. This position will require immediate patient assistance on the medical floor. The BHCC/ECM Case Manager will manage a case load of complex care clients. All clients may be engaging in behaviors that pose health and safety risks. A successful BHCC/ECM Case Manager will meet individuals "where they are at" and provide non-judgmental, non-coercive services, and resources to individuals seeking help. In addition, the role will provide consultation, education, training, and support in their work with participants through utilization of harm reduction strategies. Proficient and detailed documentation within the EHR (Electronic Health Records) system in order to meet billing requirements for the ECM program. This is a billable ECM provider position and correct monthly documentation is essential to the position.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Other duties may be assigned which may become essential to the position.
- Assist patients in establishing self-management goals using assessments and screeners including but not limited to the PHQ-9, WRAP. Blood pressures and needs assessments. Meet with patients to review and complete documentation and ECM enrollment agreements at the request of the medical team.
- Receive warm hand-offs from medical providers to assist patients with basic needs and health coaching. Use Motivational Interviewing skills when communicating with patients and de-escalation in crisis situations as needed
- Communicate with patients and the medical team when there is an adjustment in the care plan including a request to change provider.
- Job improvement training to remain current on MHSA laws, 5150 guidelines.
- Track patient appointments include outreach, scheduling new appointments and following up on missed appointments in order to assist in the coordination of integrated behavioral health.
- In house and field case management including check in calls, transportation, and patient advocacy, assistance with medication adherence and as needed home visits to assist with the coordination of care for patients.
- Assist in outreach and data collection of patient care in order accurately track and record patient progress. Help with specified grants as assigned.
- Completion of Treatment Authorization Requests TAR's, document submittal into Collective Medical and Partnership.
- Understanding of all complex care programs within the community and Hill Country.
- Collaboration with community resources to assist in patient care and support
- Able to adapt quickly to the busy and changing clinic environment
- Documentation of every client transaction into the Emergency Health Record (EHR)
- Full utilization of billing and diagnosis codes along with time units within the EHR
- Completes all other duties as assigned.
- Proficiency with EHR (Electronic Health Records) system. Detailed documentation in order to meet billing requirements for ECM program.

QUALIFICATIONS:

Education/Experience:
- High School Diploma or GED (Bachelors preferred)
- One to three months related experience and/or training 1-2 years
- Skill in communicating with patients and health professionals
- Proficiency with EHR
Certificates and Licenses:
Current CPR certification. Current CA driver's license.
Language Ability: Must have intermediate level language skills. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before individuals, groups or employees of organization.
Math Ability :
Must have basic math skills. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentages.
Reasoning Ability:
Must have intermediate level reasoning skills. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
Computer Skills:
To perform this job successfully, an individual should have good working knowledge of Windows operating systems, Microsoft Office Suite – Word, Excel, Outlook, and Internet browsers and related software. Must have good working knowledge of medical management software.
Equipment Skills:
Proficient with computers, copiers, fax machines, printers, multi-line telephone systems.
