• Conduct psychosocial assessments and care planning for residents
• Provide counseling and emotional support to residents and families
• Ensure compliance with all regulations and care standards
• Advocate for residents and help them adjust to their environment

• LSW certification in New Jersey
• Strong communication and interpersonal skills
• Compassionate, patient, and team-oriented
• Experience in long-term care or healthcare settings preferred
