Hospice Social Worker

We are looking for a social worker to support our patients and families. The perfect candidate is comfortable discussing end of life care and conducting assessments in the patient's home. Knowledge of ALTCS,VA and other community resources is a must. Hospice experience is preferred but not mandatory. Hospice reaffirms life and focuses on dignity and adds comfort and quality to our patients lives. This may become a full-time position with the right candidate.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Assist patients and families with placement and caregiving needs. Having contacts in the community for placement is important.
• Complete relevant forms as necessary (i.e. legals, Advance Directives and ALTCS applications) Familiarity with these forms is a must.
• Consult with primary care provider, and other health professionals as required.
• Provide support, education and act as a resource to our team, patients and families in order to assist them with acceptance of terminal prognosis.
• Develop and facilitate support groups as necessary for bereavement and grief.
• Maintain professional relationships with staff and referral resources.
• Complies with Compliance Plan, Complies with HIPAA Policies. Maintains confidentiality of information relating to the patient and family.
• Competent at using EMR and assisting with admission paperwork.
• Available for after hour duties/calls per rotation (generally 1 weekend every month which pays in addition)

MINIMUM JOB REQUIREMENTS:

Education:

• Bachelor's degree in Social Work required; Master's degree in Social Work is preferred.

Experience:

• One (1) to three (3) years' experience preferred working as social worker within hospice or hospital. Prefer previous experience working in hospice and with the elderly.
