Role Description

This full-time on-site Physical Therapist role is based in Emporium, PA. The Physical Therapist will assess, plan, and implement personalized treatment plans to help patients improve mobility, alleviate pain, and regain strength. Responsibilities include delivering patient care in a Skilled and long-term care facility. Collaboration with medical staff, detailed documentation of patient progress, and educating patients and caregivers on therapeutic exercises are critical components of this role.

Qualifications

• Proficiency in Physical Therapy and Rehabilitation practices
• Understanding of general Medicine principles related to therapy and patient treatment
• Licensed Physical Therapist qualification in Pennsylvania
• Strong communication and interpersonal skills to collaborate with patients, families, and interdisciplinary teams
• Ability to create and manage personalized treatment plans to achieve patient goals
• Experience in post-acute care or skilled nursing facilities is a plus
