Responsibilities

  • Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Monitor patient health by checking vital signs and observing changes in condition; report findings to healthcare professionals as needed
  • Administer medication following prescribed care plans and ensure compliance with HIPAA regulations for patient privacy
  • Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary
  • Support residents with Alzheimer's disease, dementia, or memory care needs through specialized engagement and safety measures
  • Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control protocols are followed
  • Use equipment such as Hoyer lifts or resident lifts safely to assist with mobility and prevent injury during transfers
  • Document care activities accurately in compliance with healthcare standards and assist in updating personalized care plans

Skills

  • Proven experience in home & community care, assisted living, nursing homes, or long-term care environments
  • Knowledge of patient observation techniques and ability to recognize signs of deterioration or distress
  • Certification in CPR and First Aid to respond effectively during emergencies
  • Familiarity with medication administration procedures and HIPAA regulations safeguarding patient confidentiality
  • Ability to work compassionately with individuals with developmental disabilities, autism, or dementia-related conditions like Alzheimer's or other memory impairments
  • Strong communication skills for collaborating with healthcare teams, residents, and families
  • Physical stamina for heavy lifting tasks and assisting residents with mobility needs using proper techniques and equipment
  • Experience working within care plans tailored for seniors or individuals requiring hospice or palliative support
