SUMMARY:

Purpose of the Job: Manage and supervise the Pavilion nursing staff to ensure that the medical needs of our memory care residents are met by exceeding the expectations of our residents, their families and/or responsible parties and healthcare providers while complying with appropriate rules and regulations.

Responsibilities and Tasks:

Essential Job Responsibilities

  • Consistent and predictable attendance.
  • Dispense prescribed medications to patients according to doctors' written orders. Document properly.
  • Oversee medical care of Pavilion residents: a. Recognize medical issues as well as monitor the status of chronic illnesses. b. Coordinate with external health care providers such as home health and hospice. c. Administer health assessments such as LOC, UAI, and ISP. d. Communicates any concerns to healthcare providers as well as family members or responsible parties.
  • Document daily including, but not limited to, medication administration, nursing notes, communication with healthcare providers and family members/responsible parties plus complete orders.
  • Provide direct personal assistance and supervision needed by memory care residents with their bathing, dressing, eating, walking, going up and down steps, smoking, and attending to any personal needs the residents may be unable to attend to themselves. Encourage and assist residents to care for their grooming, clothing, and toilet articles, including care of the body, hair, gums, teeth, dentures, fingernails, clothing, and personal items.
  • Respond immediately in case of an accident or incident involving a resident. Document incident.
  • Recruit, orient, retain, appraise, train, discipline, and supervise Pavilion staff including staff assignments.
  • Assist with the facilitation and coordination of pre-admission and admission activities, transfers, and discharges in the Pavilion by working with the Director of Resident Relations, Wellness Director, Executive Director, and Life Connections Program Director.
  • Conduct community tours, back-up marketing duties, and handle resident or family concerns in the absence of the Director of Resident Relations and/or the Executive Director as well as act as Manager on Duty on a regular rotation.
    1. Attend training as required as well as complete all Relias training as scheduled.

Knowledge, Skills, Abilities, or Information Required for Completely Satisfactory Performance:

  • Knowledge of principles and processes for providing resident and personal services such as conducting needs assessments and meeting quality standards of service.
  • Knowledge of principles, methods, and procedures for diagnosis and treatment of physical and mental issues with residents.
  • Knowledge of nursing care principles including symptoms, treatment alternatives, drug properties and interactions, and preventative health care measures.
    Strong knowledge of the 5 rights of medication administration- -The Right: Drug, Dose, Route, Patient, and Time.
  • Ability to tell when something is wrong or is likely to go wrong with a resident.
  • Ability to listen to and comprehend information and ideas presented by residents, family members, co-workers, visitors, and medical professionals.
  • Ability to communicate ideas and information to residents, family members, co-workers, visitors, and medical professionals.
  • Ability to make decisions and solve problems by analyzing information available.
  • Ability to perform and prioritize multiple tasks effectively and efficiently.
  • Ability to work within defined time constraints.
  • Ability to take initiative and follow through.
  • Ability to maintain accurate records and prepare reports.

Education and Licenses: Requires the ability to use fractions, decimals, and commercial arithmetic. Basic knowledge of grammar, spelling, and punctuation is required. Equivalent to 4-year high school education. A Licensed Practical Nurse license is required except if working on provision pending taking the state boards.

Experience: Minimum 1-year supervisory experience in a healthcare environment. Minimum 1 year of nursing experience in long-term care as an LPN or Med Tech.

Special Requirements:

  • Must have the ability to communicate as per section 22VAC
  • Must be considerate and accepting of the aged, frail, and disabled person.
  • Appropriate attire and good personal hygiene are expected.
  • Must comply with and implement all Company Policies and Procedures as well as the Employee Handbook.
  • Must support and promote the "Rights and Responsibilities of Residents of Adult Care Residences."
  • Must maintain confidentiality with regard to residents, staff, and Company information.
  • Must participate in training and in-service programs as required by the Company and/or applicable State Standards including, but not limited to, OSHA-required training, CPR, etc.
  • Staff will not accept gratuities from residents, family members, vendors, or visitors.
