Licensed Practical Nurse

Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care, assist with medical procedures, and support the nursing staff in delivering optimal health services. This role is essential in ensuring that patients receive the best possible care in various settings, including hospitals, nursing homes, assisted living facilities, and home care environments.

Responsibilities

• Deliver direct patient care under the supervision of registered nurses and physicians.
• Administer medications and treatments as prescribed, ensuring compliance with safety protocols.
• Conduct patient assessments, monitor vital signs, and document medical histories accurately.
• Assist with IV infusions, catheterization, and other medical procedures as needed.
• Provide support to patients with developmental disabilities, Alzheimer's care, and memory care.
• Collaborate with healthcare teams to develop and implement individualized care plans.
• Educate patients and families about health management and wellness strategies.
• Maintain accurate medical records in compliance with HIPAA regulations.
• Participate in discharge planning and case management for patients transitioning from acute care settings.
• Ensure a clean and safe environment for patients by adhering to infection control protocols.

Qualifications

• Current LPN license in good standing.
• Experience in critical care, long-term care, assisted living, or home health preferred.
• Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks is advantageous.
• Proficiency in patient assessment techniques and basic life support (BLS).

Job Types: Full-time, Contract

Pay: $ $36.00 per hour
