Schedule: Monday–Friday, 8:00 AM – 5:00 PM | No call | No weekends

Position Summary

The RN Transitions Navigator is a vital member of our hospice team who blends strong clinical judgment with compassion, communication, and community connection. This role leads the admission process for new hospice patients while also building and maintaining trusted relationships.

The ideal candidate is clinically confident, empathetic, engaging, and passionate about guiding patients and families through end-of-life transitions with clarity, dignity, and grace.

Key Responsibilities

Hospice Admissions & Clinical Care

  • Respond promptly to hospice referrals and complete nursing admission visits in homes, facilities, or hospitals.
  • Assess patient eligibility, complete required admission documentation, and collaborate with the hospice physician for certification.
  • Educate patients, families, and caregivers on hospice services, care plans, and available support.
  • Establish trust and serve as the primary point of contact during the critical admission window.

Transitions Navigation & Follow-Up

  • Communicate regularly with facility staff and community partners to support timely and appropriate hospice transitions.
  • Serve as a clinical resource to families and referral partners during periods of change or decline.

Relationship Building

  • Serve as a friendly, knowledgeable hospice presence in SNFs, ALFs, hospitals, clinics, and the community.
  • Educate facility staff, discharge planners, and families on hospice eligibility and benefits.

Qualifications

  • Registered Nurse (RN) with an active Texas license.
  • Minimum of 2 years of nursing experience (hospice or palliative care preferred).
  • Strong interpersonal, communication, and customer service skills.
  • Ability to educate, build trust, and advocate with warmth and professionalism.
  • Comfortable navigating EMRs, admission documentation, and hospice eligibility guidelines.
  • Reliable transportation and willingness to travel within a 60-mile radius as needed.
