We are looking for…

Assessments, Data Collection & Program Monitoring

  • Conduct formal and informal assessments and observations
  • Assess individuals to determine individualized habilitation needs
  • Develop and supervise the implementation of the Residential Plan /Staff Action Plan (SAP) goals
  • Monitor the integration of services and make changes in the objectives of the SAP
  • Schedule Life Plan meetings
  • Develop/Monitor/Assess monthly goals and objectives to teach the valued outcomes outlined in Life Plans
  • Write/Design and revise all individual goals and observe goal implementation by Direct Support (DSP) Professionals
  • Write and report on monthly progress notes
  • Assist in curriculum development, goal and data sheet development
  • Perform observations and spot checks to monitor the implementation of goals
  • Make changes to goals based on analyzing data
  • Complete all required documentation in accordance with required timeframes (e.g. ISP/Life Plan meetings, service tracking sheets, goal sheets, progress notes, etc.)
  • Attend ISP/Life Plan meetings (Annual review and 6-month review), monthly staff meetings, and IDT meetings
  • Follow up with staff on any errors or discrepancies in data collection

Training & Support

  • Retrain staff and revise goals as needed based on observations and clinical recommendations
  • Ensure the quality of staff in delivering and providing the necessary supports and supervision to each individual served by providing supervision, coaching, training, etc.
  • Participate in the screening of potential individuals
  • Complete staff observation and provide feedback
  • Maintain contact with the individual's family, Care Coordinators, Behavioral Intervention Specialists (BIS), parents, treatment centers, and other Elmy's programs to ensure continuity of services
  • Maintaining individual & family confidentiality is a must
  • Treat all families with dignity and respect; allow for individual differences; provide an atmosphere of acceptance and caring

Qualifications & Work Experience

  • Bachelor's Degree in one of the following human services professional fields (including, but not limited to sociology, special education, rehabilitation, counseling or psychology) required
  • 1-2 years' experience working directly with people with intellectual disability or individuals with physical and/or developmental disabilities and co-morbid psychiatric diagnoses is required
  • Proficiency in Microsoft Office is required
  • Having a clean and valid driving record and license is highly preferred
  • General knowledge of theories and methods of behavioral analysis, teaching strategies, assessments, and goal development is preferred
  • Ability to safely assist lifting individuals of various weights & 20 lb. items required
  • Commitment to company values and adherence to policies
  • Perform other duties as assigned by supervisors and/or senior management
  • Must be able to communicate effectively with others and individuals served
  • Clearance through state mandated Background/Fingerprint Check(s) is required
  • Must be able to work 1 weekend day
  • Schedule is either Sunday to Thursday or Tuesday to Saturday

Job Type: Full-time

Pay: $60,000.00 per year
